JOB SUMMARY
The Health Unit Coordinator (HUC)/Certified Nursing Assistant (CNA) is a combined role that functions under the supervision of physicians or registered nurses to provide coordination of admissions, transfers, discharges, and provides patient/family care plans based off of a collection of patient information. The individual is responsible and accountable for complete and accurate processing of information, communicating and collaborating with healthcare team members to facilitate patient care and to provide patient care for significantly achievement of patient and family outcomes. This individual will facilitate the acquisition of materials and assist with clerical activities to support the units operations and completes routine and delegated direct patient care activities communicated by the registered nurse.
ESSENTIAL JOB FUNCTIONS
1. Acquires information and processes patient related orders and performs other clerical duties to facilitate effective unit operations.
2. Communicates information effectively to team members providing patient care to optimize the team’s function and enhance productivity.
3. Coordinates processes associated with admissions, transfers and discharges to ensure proper and effective completion of the patient’s care.
4. Collects and distributes medical records as necessary to ensure the proper dissemination of patient care information.
5. Orders and maintains appropriate supply inventory for the unit to provide the unit with adequate levels of materials to be utilized at any time.
6. Coordinates and schedules appointments or procedures to ensure the patient care needs are completed in a timely manner.
7. Collaborates with direct patient care team members ensure patient care services are adjusted and completed with the patient’s changing care needs.
8. Provides direct care support and technical assistance to the Registered Nurse (RN) to allow the RN to primarily focus on and achieve the patient’s care planning and coordination needs.
9. Collaborates with the RN to establish mutual care priorities on a shift‐by‐shift basis.
10. Effectively communicates meaningful pertinent information to appropriate individuals to assist in accurate care for the patient.
11. Completes routine and delegated tasks in a timely manner and seeks assistance from the RN to prioritize activities when workload demands cannot be met to ensure the patient’s care it completed thoroughly and timely.
12. Assists the RN to complete focused technical skills and procedures in support of therapeutic and/or diagnostic interventions.
13. Assists the patient and family to complete activities of daily living, anticipates the patient’s needs and modifies the approach based on the plan of care, patient coordination, patient age and development and patient request(s) to allow growth and development of the patient and family functions.
14. Collects data and offers input on project teams to participate in process improvement activities enhancing the organization’s effectiveness.
15. Regular attendance is required in order to carry out the essential functions of the position.
16. Reviews and meets ongoing competency requirements of the role to maintain the skills, knowledge and abilities to perform, within scope, role specific functions.

JOB QUALIFICATIONS
EDUCATION
For positions requiring education beyond a high school diploma or equivalent, educational qualifications must be from an institution whose accreditation is recognized by the Council for Higher Education and Accreditation.

Preferred/Optional: Health Unit Coordinator (HUC) diploma.
EXPERIENCE
Minimum Required: None
Preferred/Optional: Experience working as a Health Unit Coordinator and a Certified Nursing Assistant (CNA).
CERTIFICATIONS/LICENSES
The following licensure(s), certification(s), registration(s), etc., are required for this position. Licenses with restrictions are subject to review to determine if restrictions are substantially related to the position.
Minimum Required: Must have or has had a Certified Nursing Assistant (CNA) certification awarded by the State of Wisconsin. Basic Life Support (BLS) certification awarded by the American Heart Association (AHA) within three months of hire.
Preferred/Optional: Basic Life Support (BLS) certification awarded by the American Heart Association (AHA) at time of hire.
