Coordinator Minimum Data Set Registered Nurse

The MDS Coordinator is responsible for the timely and accurate completion of the Federal and State assessment tool, the Minimum Data Set (MDS). Utilizes ADL’s for resident assessments; defines appropriate criteria for determining patient status; determines the level, duration, and types of patient’s rehabilitation needs. Must be able to work with OT/PT/P and other departments in care planning and coordination. Demonstrates knowledge in Medicare reimbursement mechanisms. Understands the financial concepts and implications of RUG III and PPS.

Responsibilities

Demonstrates strong assessment and patient care evaluation skills. Knowledgeable in OBRA regulations. Reviews medical records and patient care plans for completeness and compliance to State, Federal rules and regulations. Acts as a liaison to the inter-disciplinary team. Is able to input, correct, print and transmit MDS.
Completes all MDS assessments within designated time frames.
Participates in planning and adjustment of Medicare assessment scheduling.
Validates the entire MDS once printed and ensures documentation is present to support the MDS.
Writes RAP summaries.
Assists with and teaches care planning process to nurses and other members of the team.
Completes and maintains Resident Roster and Census.
Responsible and accountable for the scheduling completion, accuracy and timeliness of the MDS and care plan process.
Responsible for the timely transmission of the MDS.
Performs other duties consistent with purpose of job as directed.

Required

Maintains cooperation with other departments and within own department.
Graduated from an accredited school of nursing as an R.N.
Currently Licensed in the State of Michigan
2 years prior experience with the MDS process.
