Job Summary
The Nurse Navigator will assist in the ongoing care of patients and families with cancer and will see patients in both the inpatient and outpatient settings and will work with a multi-disciplinary team to help coordinate care. The ideal candidate has a strong work ethic, is a great team player, communicates well and is organized.
The Nurse Navigator functions as a member of the multidisciplinary team as a single point of contact for patients from point of entry, through diagnostic studies and treatment plan for cancer through survivorship or end of life. The Nurse Navigator's primary function is to eliminate barriers and ensure timely access to care while building a relationship with patients and team members, to coordinate a plan of care including appointments, transportation, education, provision and/or enablement of support services and representation within the multidisciplinary care environment. The position encompasses the roles of coordinator, liaison, communicator, educator and supporter for the patient and family. The Oncology Nurse Navigator contributes to the productive and efficient operations of the healthcare system.
The Nurse Navigator also assumes responsibility and accountability for the management of resources to achieve efficient, high quality outcomes for each cancer patient including support for interdisciplinary and cross facility collaboration and communication.
The Nurse Navigator will serve as a liaison between the patient and family, primary care physician, internal and external care providers, specialists, referrers, support network members, and the healthcare community.
The Nurse Navigator will track performance of the program in line with facility and program goals, , and identify opportunities to streamline care practice.
Qualifications/Licensure/Certification
- Current licensure as a registered nurse in the State of Ohio
- Bachelor’s Degree in Nursing
- American Heart Association BLS, ACLS
- Three years acute care clinical experience and clinical proficiency, clinical oncology experience a plus
- Excellent customer service, communication, and interpersonal skills
- Demonstrated leadership and teaching abilities
- Professionally appropriate analytical and organizational skills
- Ability to work in fast-paced environment and manage/prioritize/delegate patient care and complete work assignments and other duties as assigned
- Ability to speak fluent Pennsylvania Dutch a plus
Essential Responsibilities/Functions
The Oncology Nurse Navigator implements and supports the philosophy, mission, vision, values, standards, policies and procedures of Pomerene Hospital.
- The Oncology Nurse Navigator performs care coordination core functions which include:
- screening and identification of patients
- establishment of an effective relationship with patient/family
- assessment of patient/family strengths and needs
- development of a family-centered plan of care
- procurement and coordination of services
- monitoring and evaluation of progress
- Facilitates the patient and family’s ability to assume responsibility for managing their own health care when appropriate by:
- promoting return of the patient/family to the highest level of wellness possible
- identifying appropriate services by appropriate resources at the appropriate time
- assisting in the acquisition or verification of financial /payer/insurance information when appropriate
- managing communication from the patient/family in the area of their expertise in between visits, hospitalizations
- Facilitates and coordinates quality patient care in a timely and cost-effective manner while promoting multidisciplinary collaboration between all members of the health care team.
- Acts as a resource to members of the health care team in providing patient, family, and staff with information regarding diagnosis, treatments, side effects, and current therapeutic trends in specialty area.
- Coordinates and mobilizes health care and community resources to meet the needs of patients and their families through referral, consultation, education and/or care conferences.
- Collaborates with members of the health care team in identifying learning needs, planning for, and providing instruction in the care of the patients and their families across the continuum
- Serves as a resource and liaison between Pomerene Hospital, Aultman Hospital, other outside facilities and the community regarding health care information in the area of specialization.
- Maintains accurate documentation in the medical record of assessments, patient plans, interventions and patient response to intervention, and education provided to patients/families.
- Serves as a patient/family advocate in meeting patient or family needs.
- Actively pursues education and knowledge related to the Oncology nursing profession and their area of expertise.
- Identifies need for plans and/or provides continuing education programs in specialty area and develops/coordinates appropriate educational resources.
- Facilitates and coordinates ongoing educational needs of patients/families.
- Facilitate and assist in planning community programs for cancer service line including prevention, screening and education
WORKING CONDITIONS:
- Hours may be flexible to meet the needs of patients and include evening or weekend community outreach events
