Registered Nurse RN Qualifications:
- Graduate of state-approved school with course in pharmacology
- Reliable, independent means of transportation to make home visits. Drivers must have a valid driver’s license and drive a vehicle with insurance limits as established by the state.
DUTIES AND RESPONSIBILITIES for the Registered Nurse RN Position:
- Provides skilled nursing care that includes activities such as teaching, treatments, (including IV therapy, venipuncture, complex wound care) and health promotion practices
- Executes preventive, rehabilitative, and restorative nursing procedures appropriate for the patient’s care and safety
- Assesses signs and symptoms in conjunction with reporting reactions to treatments, including drugs, and changes in patient’s physical, mental, or emotional condition to the physician
- Develops the plan of care collaboratively with the physician, patient, designated representative, and the agency with revisions made based on patient condition
- Instructs and counsels the patient and the designated representative regarding the care requirements of the patient in the home
- Assigns and instructs the Licensed Practical Nurse (LPN) or home health aide in accordance with the physician’s plan of care
- Supervises and evaluates the care given by the home health aide to his/her primary patients every 14 days
- Supervises and periodically evaluates the care given by the LPN to his/her primary patients to ensure oversight
- Evaluates and manages LPNs and home health aides who are providing services to his/her primary patients
- Obtains a medical history, particularly as it relates to the present condition, from the patient and/or designated representative at the start of care.
- Conducts a comprehensive physical examination and nursing assessment of the patient at each skilled visit
- Evaluates the patient and the home situation to determine the health teaching required at each skilled visit
- Evaluates the patient’s condition and home situation to determine what assistance will be available from the designated representative in caring for the patient
- Coordinates total patient care serving as the primary nurse for designated patients and evaluating the effectiveness and the appropriateness of care
- Interprets nursing and other services of the agency to the patient and designated representative as part of planning for care
- Serves as the primary agency liaison and coordinator of care when nursing and other services are provided
- Plans with the patient, designated representative, and other team members for the patient’s discharge from service
- Meets with the interdisciplinary team routinely to ensure coordination of care and to update the plan of care as appropriate
- Meets with Clinical Supervisor or Case Manager weekly for an evaluation of patients under his/her care
- Completes all appropriate OASIS assessments and documentation as assigned
