The Data Abstractor has responsibilities for the collection and coding of patient demographics, cardiac risk elements, definitive treatment, extent of disease, and patients’ disease status from a variety of source documents, for state reporting, National Cardiac Data requirements (NCDR), STS, GWTG, VQI, etc. The Data abstractor is accountable for the efficient operation with regard to timeliness, accuracy, and complete data collection. Data abstractors are essential in the evaluation and improvement of patient care.

JOB DUTIES AND RESPONSIBILITIES:

Maintains a current knowledge of the abstracting guidelines, guidelines for data entry, and specification manual updates and revisions for registries as assigned.
Determine class of case through review of source documents according to the New York Heart Association classification system. Notifies the physician if elements are missing from the chart.
Utilizes computer expertise in several different computer systems to access information necessary for case completion, including but not limited to Care Manager, HPF, HBI, Excel, and the Centricity data base.
Sends emails and makes phone calls to obtain complete diagnosis and treatment information from the physicians.
Maintains a working knowledge of the computer systems and keeps current on all updates.
Electronically submits inclusive data to the registries as assigned by the databases.
Establish work priorities and effectively balance varying workloads.
Compiles reports and sends an Executive Summary to the Service Line directors and selected administrators per specific registries.
Attends educational activity annually based on specific registries.
Actively participates in Performance Improvement functions.

PHYSICAL AND SENSORY REQUIREMENTS:

Sitting for up to 7 hours a day, 3 hours at a time. May require occasional standing and walking for up to 2 hours per day. Extended periods of vision use in reviewing computerized patient records and repetitive arm/finger use in retrieving and abstracting of pertinent information into computerized database. Uses upper extremities to occasionally lift and/or carry up to 25 pounds. May require stooping, bending, and/or reaching above shoulder level. Hearing as it relates to normal conversation.

EDUCATION:

RN required, BSN preferred. Will consider at least 2 years of recent experience in a patient care especially focused on stroke care.
