People deserve thoughtful solutions that put their personal care needs at the center, proactively educate and connect them with resources, and help manage the confusion and complexity of downstream care. To deliver this type of highly personalized, hands-on care requires time most providers currently don’t have and is financially difficult. Our new Advanced Primary Care (APC) model seeks to change the status quo. Combining modern clinics with modern technology to deliver high-quality, easily accessible, team based care across in-person and virtual primary care, integrated with urgent care.

Training & Experience
- A Master's degree and certification by Physician Assistant or Nurse Practitioner certifying body is required.
- Clinical experience in family medicine, internal medicine, or ambulatory care setting is preferred.
- BLS certified
- Must have and maintain a DEA license with no restrictions.
Knowledge Of:
- The principles and processes for providing customer and personal services. This includes customer needs assessment, meeting quality standards for services, and evaluation of customer satisfaction.
- Applicable federal, state and local laws and regulations including the requirements of the HIPAA and OSHA and applicable state rules and regulations pertinent to medical practices.
- Computerized information systems used in business applications and clinical management systems (EMR).
- Relevant drugs and their chemical composition, structure, and properties of substances and of the chemical processes and transformations that they undergo.
- Non-pharmaceutical patient care aids, their applicable uses, and relevance to patient care.
- Current principles, methods and procedures for the delivery of medical evaluation, diagnosis and treatment.
- Accreditation and certification requirements and the ability to maintain all appropriate licenses.
Ability To:
- Perform duties in accordance with the customary rules of ethics and conduct of the applicable State Board of Nursing, State Board of Medicine, and other such regulating bodies.
- Prescribe dosages and instruct patients in correct usage. This includes uses of chemicals and their interactions, danger signs, production techniques, and disposal methods.
- Respond to requests and inquiries from the general public in person and over the telephone.
- Interpret the application of OSHA and HIPAA laws. Maintain quality, safety, and infection control standards.
- Exercise tact and diplomacy in dealing with sensitive, complex and confidential patient issues and situations.
- Establish and maintain effective working relationships with those contacted in the course of work.
Expectations:
- Nurse Practitioners or Physician Assistants shall work collaboratively and pro-actively with other health care practitioners at the APC Clinics, and with patients, families, and others to efficiently treat patients, and plan coordinated, efficient, and thoughtful dispositions (including transfer), all in a timely manner and in cooperation with APC Clinics staff.
- Is comfortable working in a team based environment. The individual will strive to deliver holistic care to patients working in a coordinated manner with a care team. The care team comprises of a clinical pharmacist, nurse, health coach, behavioral health specialist, and medical assistant.
- The individual shall be responsible for the preparation of a clear, complete, accurate, and legible medical record for each patient under her care. They shall type, edit and electronically sign all office notes, reports and records using the APC Clinics’ systems in accordance with the APC Clinics’ policy and in a timely fashion.
- They shall devote such additional time as necessary to attend to the APC Clinics’ patients, conduct dictations, complete paperwork related to her clinical duties, and perform administrative duties hereunder.
- Will work collaboratively with other ConvenientMD-employed practitioners to develop daily work schedules, vacation time, and on-call schedules for the APC Clinics.
- Shall fully cooperate with ConvenientMD in efforts to develop the APC Clinics’ services as needed to meet community needs and to make practitioners and community members aware of the APC Clinics and its benefit to the community. Shall represent and promote the APC Clinics’ practice as appropriate in all public and professional meetings, conferences, other professional activities, and in all interactions with the public and other health care practitioners and staff.
- Shall conduct themselves in a professional manner; communicate with others in a timely, respectful manner; and otherwise cooperate with colleagues and staff to facilitate high quality, efficient patient care. Shall adhere to the APC Clinics’ policies on attire, hygiene, and grooming and shall consistently present themselves to others in a professional manner.
Physical Requirements:
- Requires the ability to stand and sit for hours at a time. Some bending and stooping is required.
- Must be able to use manual dexterity in relation to clinical requirements.
