What You'll Be Doing
The licensed practical nurse is expected to perform tasks and responsibilities within the framework of nursing services and the scope of practice in a skilled nursing facility. The LPN is part of the interprofessional team to help lead the resident and family teaching program through health teaching, health counseling, provision of supportive and restorative care, under the direction of a registered nurse or licensed clinician/provider.
- Provide comprehensive care to patients by applying the nursing process in an organized and systematic manner according to policies of the Medical Center and The Transitional Care Unit in keeping with patient safety principles
- Performs all clinical skills and services as per policy and accepted procedure within the LPN scope of practice
- Completes clinical competency, required certifications, and peer evaluation process
- Collaborates with the resident, significant others, ancillary departments, and functions as part of the multidisciplinary health care team whose goal is to provide care that is continuous and well-coordinated with specific discharge goals
- Performs effectively and efficiently in emergency situations
- Evaluates and documents resident progress/response toward expected outcome(s) and reports pertinent observations and recommendations to the interprofessional team
- Communicates proactively to the interprofessional team the plan of care changes in patient condition, the patient progress, the discharge plan and other relevant information
- Documents resident care and required safety checks accurately and legibly
- Performs ongoing evaluation of residents’ physical and mental health and notifies the interprofessional team and/or clinician/physician, including abnormal lab and diagnostic results
- Explains medications, procedures, and treatment protocols to residents
- Administer medications via intravenous, oral, topical, subcutaneous, intramuscular, gastrostomy and other route of administrations within scope of practice
- Administer enteral feedings via nasal gastric tube or gastrotomy tube
- Perform bedside care such as wound dressing, changing bandages, inserting catheters, and giving injections
- Obtain resident's vital signs such as respiration, temperature, blood pressure, and heart rate as needed and documented
- Supervise certified nursing assistants and other auxiliary staff
- Taking verbal orders from the provider/physician and document appropriately
- Notifies physician of any pharmacist consultant’s recommendation for revision of medications, medication alerts, or changes in medication administration times as written in the Electronic Pharmacist Information Consultant (EPIC) form then implements the approved changes
- Initiate and/or discontinue peripheral IV heplock
- Call pharmacy for ordering prescriptions when discharging residents home
- Provides hand-off report to receiving unit when transferring patients
- Maintains a safe and comfortable environment for residents, visitors, and staff
- Complies with policies and procedures concerning infection control and safety for all individuals on the unit
- Maintains and develops competence through required in-service and continuing educational programs
- Maintains current CPR and IV certifications
- Discharge patients to home or other healthcare settings as supervised by a registered nurse
- Active and contributing member of the Quality Assurance Performance Improvement Committee
What We're Looking For
Full-time, Night shift, Rotating weekends, every other holiday, 12 hour shifts
- Active NJ LPN license and BLS certified.
- Preferably a minimum of 2 years’ experience in post-acute care settings (i.e. skilled nursing facility, rehabilitation, long-term care).
