POSITION SUMMARY
The clinical manager is a licensed health care professional that supervises and coordinates staff to ensure provision of quality patient care and effective team performance to meet agency goals in accordance with philosophy, policies, mission and vision.
QUALIFICATIONS
- Registered nurse with a current valid Pennsylvania license BSN preferred or physical therapist, occupational therapist, or speech-language pathologist with a current valid Pennsylvania license.
- Two [2] years home health experience with roles reflective of progressive responsibility, including supervision in home health.
- Knowledge base of agency policies, federal and state regulations and third party payers
- Exhibits strong organizational, communication and interpersonal skills. Evidence of leadership activities
- Reliable, independent means of transportation to make home visits. Drivers must have a valid driver’s license and drive a vehicle with insurance limits as established by the state
- Experienced with computers and electronic health record software/documentation.
DUTIES AND RESPONSIBILITIES
Clinical Skills
- Coordinates staffing patterns to meet staff and geographical needs among staff; take and coordinate referrals as necessary to ensure agency coverage
- Maintains schedules for all staff for daily, week-end and on-call coverage
- Handles phone calls and messages to and from staff, patients, families and referral sources
- Accountable for clinical staff and provision of appropriate clinical services to patients serviced by the agency
- Interview applicants and assist in hiring decisions to maintain a qualified staff in sufficient numbers to serve patient needs
- Assures that patient needs are continually assessed and assists with consulting physicians for orders and care plan development, implementation, and updates.
- Maintains clinical skills by providing hands on care to clinically complex patients
- Applies problem solving abilities to specific organization/clinical activities
- Works with the Compliance and Ethics Manager to assure regulatory and certification compliance at local, state and federal levels
- Participates in Utilization and Quality Improvement activities
- Identifies need for in-services for the staff and assists with in-services, as needed
- Provides back up to the Clinical Supervisor for the review of Plans of Care for accuracy and completeness and signs the 485 before submitted to the physician for signature, if needed
- Provides back up to the Clinical Director, as needed
- Assists in supervising all patient care activities to assure compliance with current standards of accepted nursing practice
- Assists with competency testing for nurses and home health aides
- Performs performance reviews of the clinical staff with input and review as needed from the Clinical Director or Branch Manager
- Monitors and maintains productivity reports
Documentation
- Assures that clinical records are kept current and in accordance with the agency’s policies and procedures
- Performs random reviews patient records for completeness and appropriate documentation
- Directs the clinical record system including clinical aspects of the billing process
Communication
- Obtains and delivers information to others by phone, fax, written reports, letters and in person
- Interacts with field staff, patients, families, physicians, other health disciplines, agencies, insurance companies, hospitals, labs, employers, government agencies and representatives as necessary
- Maintains an open door policy to resolve patient and staff concerns
- Assists the Clinical Director or Branch Manager with the organization and implementation of staff meetings as needed
- Assists the Clinical Director or Branch Manager in representing the agency on clinical matters in the community as needed
- Communicates all concerns regarding field staff, patients, families, physicians, other health disciplines, agencies, insurance companies, hospitals, labs, employers, government agencies and representatives to the Clinical Director and/or Director.
