The mission of the Colorado Coalition for the Homeless is to work collaboratively toward the prevention of homelessness and the creation of lasting solutions for homeless and at-risk families, children, and individuals throughout Colorado. The Coalition advocates for and provides a continuum of housing and a variety of services to improve the health, well-being, and stability of those it serves.

The Colorado Coalition for the Homeless seeks a ACT Nurse Case Managerto provide health assessment, counseling, and medication management to promote recovery, wellness, and optimal health. The position provides services in the community within the framework of core ACT operating principles, professional nursing ethics, and State and City regulatory guidelines. This is a field-based position and requires at least 80% of services to be provided to consumers in their homes, shelters, primary healthcare providers, and other community settings. This position also provides direct nursing medical services and mental health clinical assessment and treatment to Housing First consumers.This position is located at 2100 Broadway, Denver, Colorado 80205 and reports to the RN Manager.
Qualified candidates are eligible for the NURSE Corps Loan Repayment Program which offerspayment of60% of unpaid nursing education debtin exchange for a two-year commitment at a NURSE Corps approved sitewith an option to extend to a third year for an additional 25% of the original balance.
NOTE: In order to ensure that our consumers receive the best possible care the candidate chosen to fill this position will be required to complete our internal credentialing and privileging process prior to a start date. The credentialing and privileging timeline is dependent on the selected candidate's submission of documents necessary for clearance.Please note that a start date will be scheduled once credentialing and privileging is complete.

Essential Job Functions
- Provides basic health and medical assessment and education.
- Assists with coordination of health care provided to consumers in the community.
- Performs psychiatric and medical assessment, treatment, and education.
- Provides 24-hour on-call phone support for HF consumers on a rotational basis.
- Performs treatments and laboratory procedures independently and/or as requested by the health care provider.
- Rigidly adheres to Universal Precautions per CDC guidelines and OSHA regulations.
- Rigidly adheres to HIPAA regulations per standards developed by the Department of Health and Human Services.
- Provides appropriate patient education that is tailored to both the individuals learning style, education and/or reading level, and is sensitive to their health beliefs.
- Assesses and monitors consumers ongoing psychiatric, medical, and social needs.
- Oversees medication monitoring and medication administration under the supervision of the designated provider.
- Collaborates with internal and external providers to coordinate psychiatric treatment with medical treatment; acts as a psychosocial educational resource by assisting consumers in learning about their mental illness, physical health conditions, medications, and how to protect their overall health.
- Manages pharmaceutical and medical supplies.
- Makes home visits and delivers medications.
- Administers intramuscular medications.
- Monitors clients vital signs.
- Coordinates providers orders and medication refills.
- Maintains accurate and timely clinical records consistent with CCH standards.
- Participates in team meetings, in-services, and supervisory sessions as required.
- Must demonstrate competency in agency databases including HMIS and EHR.
- Record all services in a timely and complete fashion in the EHR
- Maintain up to date records in EHR
- Record telephone and other communications as needed in the EHR
- Manage tasks and scanning as needed to have a complete and current chart
- Maintain accurate information regarding clients insurance status
- Utilize the EHR and any future upgrades as directed

Qualifications Summary
- Graduation from an accredited school of nursing.
- Licensed as a BSN or RN in the State of Colorado.
- Certification in Basic Life Support/CPR.
