1._____ Patient Assessment/Reassessment
- Contributes to the initial patient assessment, reassess patient’s condition based upon observation and/or interview of patient, physicians’ diagnosis, laboratory findings and any other pertinent information. Reports changes to the Registered Nurse.
- Under direction of a registered nurse, contributes to nursing care plans consistent with assessment of patients’ condition and physicians’ treatment plans.
- Directs non-profession personnel in implementing nursing care plans, and provides guidance and demonstrates appropriate methods, as necessary, to correct deviations from established standards.
- Under the direction of a Registered Nurse, evaluates patient’s response to nursing care and modifies or revises treatment plans, and/or contacts physicians as circumstances require.
- Considers age and cultural/spiritual differences when assess, planning, and evaluating care.
2._____ Care of Patient
- Accurately administers medications according to the 5 rights of medication administration and in accordance with KCH policy.
- Renders nursing care including administering hospital approved medications, initiating and maintaining intravenous fluids, and monitoring blood products according to hospital policy and procedures.
- Understands the pharmacology and recognizes the side effects of medications administered.
- Under the direction of the registered nurse, perform both complex and routine treatments as prescribed by physicians or as warranted by patient’s condition.
3._____ Management of Information
- Observes patient and collects data.
- Documents observations in the patient’s record according to KCH policy/procedures
- Adheres to KCH IS Security and HIPPA policies. “The associate will hold in confidence any and all patient information communicated or stored using any medium including oral In addition, the associate will seek to protect patient information from inadvertent use or disclosure by adhering to hospital and Triad Blue Book policies.”
- Contributes to the collection and evaluation of data for the nursing Quality Review Program
- Communicates with other hospital departments, as necessary, in order to resolve operational problems and make the most effective use of hospital resources in delivery quality patient care.
4._____ Pain Management
- Assesses and reassess for pain while considering religious and cultural beliefs.
- Provide interventions and documents according to KCH politics
5._____ Cardiopulmonary resuscitation
- Initiates code response and manages the patient, personnel, and demonstrates proper use of equipment during resuscitation efforts.
- Documents events of code according to KCH policy
- Has completed CPR annual training
6._____ Patient Safety
- Implements procedures for security, equipment safety and infection control.
- Identifies reports and manages actual and potential risks to patient safety.
- Applies learning from sentinel event alerts and root cause analyses.
7._____ Patient/Family Education
- Assesses learning needs identifies barriers to learning, cultural differences and uses appropriate teaching methods.
- Instructs patients and family members regarding treatments, surgical procedures, nursing plans and so forth in order to allay apprehensions; explains home care methods and techniques necessary for continuing patient care and rehabilitation.
- Documents patient/family education appropriately.
8._____ Patient’s Rights/Abuse
- Abides by policies governing patient rights and responsibilities
- Can lists signs and symptoms of child, spouse and elder abuse.
- Documents witnesses or suspected abuse and notifies appropriate authority according to KCH policy.
9._____ Restraint Use
- Assesses need for restraint and attempts alternative mechanisms.
- Applies restraint appropriately
- Acquires physician orders for restraint as required by hospital policy
- Documents assessment according to hospital policy

11 Assists in orienting and training student nurses and new personnel, and serves as a resource person for departmental personnel resolving patient care problems.
- Floats to other units as assigned.
- Responsible for knowing the lifting requirements of the job and requesting assistance when appropriate.
- Contributes to Performance Improvement activities at Kosciusko Community Hospital
- Maintains knowledge of current trends and developments in the field by reading appropriate books, journals and other literature and attending related seminars, conferences, etc.
- Contributes to Performance Improvement activities at Kosciusko Community
Hospital.
NON-ESSENTIAL FUNCTIONS:
Performs other duties as assigned/required.
JOB QUALIFICATIONS:
Education and Formal Training Required:
Must be a high school graduate with Licensed Practical Nurse degree.
Experience Required:
Approximately three to six months on the job experience necessary in order to acquire and effectively utilize knowledge of unit and departmental policies and procedures, and to fulfill established quality and quantity standards for the job.
Licensure Required:
Currently licensed in the State of Indiana as a Practical Nurse.
Other:
Good verbal communication skills necessary in order to instruct and comfort patients and their families, maintain effective contacts with a variety of Hospital personnel; and written skills to complete documentation.
Must demonstrate knowledge and skills necessary to provide care appropriate tot he age of the patient (infant, child, adolescent, adult, or geriatric) served on assigned unit/department.
WORKING CONDITIONS:
Exposure to contagious disease through contact with patients.
