Job Description
The Clinical and Quality Improvement (CQI) Nurse is both certified as Registered Nurse (RN) and Public Health Nurse (PHN) and performs public health nursing services requiring professional skill, knowledge of the community and its resources, independent judgment and initiative. Supervision is received from the Nurse Manager and involves review of completed work, regular conferences and discussions of problem cases.
CQI Nurse functions as nurse consultant and provides nursing/clinical consultations to adults and older adults with disabilities/caregivers/family members, interdisciplinary team members being served by various programs within Department of Disability and Aging Services (DAS) programs.
ESSENTIAL DUTIES AND FUNCTIONS
According to Civil Service Commission Rule 109, the duties specified below are representative of the range of duties assigned to this job class and are not intended to be an inclusive list; may include additional duties as assigned.
- Conducts in person assessments in the community including homes, Single Room Occupancy (SRO), hotels, Skilled Nursing Facilities (SNF) and hospitals to render public health nursing services in assessing, evaluating and developing community service plans for adults and older adults with disabilities who were referred to CQI by DAS social workers.
- Interprets medical orders including medication regimen and treatment plans; explains nutritional needs, hygiene and safety measures; advises clients/caregivers regarding health promotion, disease management and crisis managements;
- Coordinates and facilitates assessments and individualized service planning to prevent premature institutionalization and hospital readmission.
- Explains how to obtain emergency medical assistance as part of disaster preparedness and emergency response; inspects and observes access and functional needs of older adults and adults with disabilities.
- Renders direct nursing services through clinical assessments and evaluations of vulnerable adults and adults with disabilities who are chronically ill, with behavioral health care needs; obtains medical care as needed; refers cases to appropriate resources; maintains nursing supervision of referred cases and keeps appropriate records of nursing and other services rendered.
- Renders nursing services, care and education in connection with the control of infections and non-infectious diseases; interviews cases and those suspected of active infections in their homes, interprets medical advice in regard to care and treatment and instructs clients how to assist themselves. Assists in infection control education and training all DAS staff and community as needed.
- Applies concepts of community organization in working with communities to meet their health needs and refers clients and families to proper agency for financial assistance, casework service, and other human services for continued client care.
- Prepares and maintains individual case records of clients and cases in family folders; uses related forms for referrals, consent reports, narrative and statistical records and for other purposes; prepares various statistical information such as clinic attendance, tests and immunizations given and other pertinent records.
- Assists with health promotion/risk reduction intervention and provides health teaching, screening, advocacy, and counseling to DAS clients, including high risk populations; triages referrals to CQI Nurse Consultation by DAS programs.
- Participates in quality assurance and improvement activities to promote efficient and effective delivery of services.
- Triages referrals to CQI Nurse Consultation by DAS programs.
- Demonstrates experience and expertise in performing age-appropriate psychosocial and functional assessments.
- Participates/collaborates in the design of CQI initiatives.
- Participates in the development/identification of standards and appropriate tools related to nursing care.
- Serves as a liaison between the social workers and licensed medical professionals in obtaining medical information; coordinates with primary medical provider/healthcare provider to ensure the connection with medical service providers who will address healthcare needs of the client.
- Participates in nurse workforce enhancement through introduction of CQI nursing assessments and interventions to nurse interns.
- Collaborates and communicates effectively with clients, appropriate interdisciplinary team members in addressing the identified nursing and clinical needs of the referred clients.
- Maintains clinical competencies and expertise in the provision of community health care plan for older adults and adults with disabilities.
- Follows the established policy and procedures relating to Health Insurance Portability and Accountability Act (HIPAA).
- Participates in teams/committees/workgroups to promote skills and knowledge related to public health priorities.
- Uses concepts, knowledge, and evidence of the social determinants of health in the delivery of services to clients.
- Manages work time effectively, efficiently, and productively.
- Performs other duties as required.
Qualifications
1. Experience: One (1) year (equivalent to 2,000 hours) of verifiable experience as a Registered Nurse in an acute hospital, primary care facility, home health agency, and/or public health agency within the last five (5) years;
AND
2. License and Certification:
a. Requires possession of a valid permanent or temporary license as a registered nurse issued by the California Board of Registered Nursing;
AND
b. Possession of a valid Public Health Nurse Certificate issued by the California State Department of Health;
AND
c. Possession of a valid California driver’s license.
