Job Summary
Under the direct supervision of the Nursing Manager, and in collaboration with the interdisciplinary team, the Registered Nurse uses clinical judgment skills to access and implement the treatment plan for patients. The RN assesses monitors, detects, plans and takes appropriate action to prevent physiological complications associated with specific health problems and/or medical treatment.
Our Visiting Nurses Association is our Home Care Skilled Nursing Team for West-Central areas of Michigan, such as Kent County and surrounding counties. We are accepting applications from experienced RN’s, LPN's, and New Grads! For experienced RN’s this is an autonomous position that drives you to use all of your top nursing skills and critical thinking. As a new graduate you will have the opportunity to train and work closely with other RNs for 6 months before working more independently.

Visiting Nursing Association a great place to begin a home health career. We offer great daytime work schedules including working every 4th weekend, working every 3rd holiday and on call 1-2 days in a 6 week rotation. 8, 10 and 12 hour shifts available. Working in home health enables you to treat the patient in the comfort of their home, where patients are able to thrive and own their own health care. We create personalized assignments close to home that enable our nurses to visit between 5-7 patients per shift.
From Ludington, to Baldwin, to Nunica and south through Kent County, Ottawa County and as far south as Gun Lake, our Corewell Health West, Visiting Nurses Team goes the distance to provide outstanding patient care.
Essential Functions
- West Michigan Trusted provider of home care services for more than 120 years
- Services Include: Rehabilitation, Heart Care, Orthopedic Care, Wound Healing, and Pediatric Care
- Offering everything patients need to stay comfortable, safe, and as healthy as possible in their home
- Communicates effectively with staff, residents and families. Assesses, develops, and maintains an individualized patient plan of care with the patient.
- As needed, specified by SH entity: Performs Medication Administration Pass according to regulatory guidelines. Informs MDS coordinator of significant resident/patient changes needed for completion of the MDS.
- Communicates with other members of the health care team in order to ensure continuity of care and coordination of services. Acts as a patient/family advocate.
- Evaluates patient’s and significant other’s educational needs and develops and implements appropriate teaching strategies or makes appropriate referrals.
- Observes and reports the patient’s condition, needs and outcomes of intervention to the physician and other team members. Identify problems and emergency situations, and initiate immediate “life saving” measures in the absence of a physician.
- As needed, specified by SH entity: Teaches and supervises the care provided by CNA, LPN or other nursing service personnel according to regulatory and agency guidelines.
- Implements the plan of care by providing nursing services, including teaching, treatments, diagnostics, and preventive procedures, regularly evaluating the patient’s progress toward established goals recommending revisions to the plan as necessary, and planning for termination of service or discharge. Assumes a leadership role and delegates appropriately to support staff to facilitate safe, quality patient care in a cost-effective manner.
- Participates in ongoing process improvement activities through the application of clinical paths, QA processes and other related activities. Ensures that processes and services are continuously monitored for quality, cost effectiveness, and efficiency.
Qualifications
- Required Associate's Degree Nursing
- Preferred Bachelor's Degree Nursing
- 1 year of relevant experience of acute care, emergency, or rehab facility experience for all of the Home Care business lines Preferred
- LIC-Registered Nurse (RN) - STATE_MI State of Michigan Upon Hire required
- LIC-Driver's License - STATE_MI State of Michigan Upon Hire required
- CRT-Basic Life Support (BLS) - AHA American Heart Association VNA and Home Care Business lines 90 Days required Or
- CRT-Basic Life Support (BLS) - ARC American Red Cross VNA and Home Care Business lines 90 Days required
