We are seeking a highly skilled and compassionate Registered Nurse (RN) in the Albuquerque/Santa Fe, NM area to join our team making a meaningful impact on patients' lives. Resource nurse would spend 2-3 days per week working out of our Santa Fe office. Minimum of 1 day required to work out of the Albuquerque office. #INDLIC

Job Responsibilities:

The Resource Nurse will be responsible to complete patient assessments, care plan review, planning and coordination of care. Provide education to the patient, their family members, and/or their home health aide regarding their conditions and disease progression. The resource nurse will also provide support to the Director of Nursing, and Compliance Manager.

The Resource Nurse will collaborate with the patient, their family, their home health nurse, home health aide, as well as the patient coordinator to ensure appropriate plan of care, execution of care plan, utilization of services, while also providing patient with community resources/recommendations.

Requirements:

RN License – Active and valid in the state of practice

Experience in Home Healthcare – knowledge of home health care plans, appropriate tasks/interventions to be completed by RNs, LPNs & Home Health Aides.

Willingness to Travel to Patient Homes – Ability to travel to various patient homes, including remote or rural areas in Northern New Mexico on an as needed basis if the patient/family is unable to present in office.

Strong Clinical Skills – Ability to manage patient care in a home setting, with effective assessment and interventions.

Preferences:

Experience with Diverse Patient Populations – Ability to handle a variety of medical conditions and needs.

Ability to Work Independently – Comfortable managing patient care without direct supervision.

Comfortable Navigating Remote Areas – Ability to navigate less-developed or difficult-to-access areas.

Strong Organizational Skills – Ability to handle multiple patients and appointments efficiently.

Cultural Sensitivity – Understanding and respect for the cultural practices and needs of reservation communities.

$80,000 - $100,000 a year

Up to $4000/quarterly bonus based on meeting performance metrics.
