Transcend Hospice & Palliative Careis seeking a Certified Home Health Aide (CHHA) who will work in collaboration and under the supervision of a Registered Nurse Case Manager. As a Certified Home Health Aide, you will put your skills into working to support daily activities, including assisting patients with getting in and out of bed, dressing, grooming, hygiene, feeding, assistance with ambulatory transfers, and light housekeeping at the end of life. This role requires a Certified Home Health Aide to travel to our service areas of Salem and Gloucester counties.

JOB RESPONSIBILITIES:

Provides personal care services to patients.
Applies safety principles and proper body mechanics to the performance of specific personal and supportive care techniques such as ambulation of clients, transferring clients, assisting with the normal range of motion and positioning, and doing household chores.
Ability to work and collaborate under the direction of the Registered Nurse Case Manager.
Maintains a clean and comfortable environment.
Observe, report, and document any changes in the patient's status. If unable to perform a specific task, report to the primary nurse.
Provides emotional support to the patient and family unit
Follow specific written instructions for personal care prepared by a registered nurse in the plan of care.
Other duties as assigned.

QUALIFICATIONS:

High school diploma or GED
Graduate from an approved training and competency course.
Must meet federal Home Health Aide training and competency Requirements.
Ability to work independently and make accurate and quick judgments.
Ability to respond appropriately to crises outside of a hospital setting.
Acceptance of and adaptability to different social, racial, cultural, and religious modes.
Must have a valid Certified Home Health Aide license for New Jersey.
Active patient contact within the past three years is preferred.
Must have a valid driver’s license, auto liability insurance, and reliable transportation. You must be willing to travel throughout the service area.

Work Schedule: Monday- Friday 8:30 a.m.- 4:30 p.m.
