Location: Brandon, Florida
Shift: Full-Time
Salary: Up to $85,000 per year

Job Description:
Victoria Crossing Rehabilitation Center is seeking an experienced and compassionate RN MDS Coordinator to join our dedicated clinical team. The MDS Coordinator will be responsible for managing and coordinating the completion of the Resident Assessment Instrument (RAI), including the MDS, in accordance with applicable federal and state guidelines. This role plays a critical part in ensuring our residents receive high-quality, person-centered care.

Key Responsibilities:

Lead the development and completion of MDS assessments, ensuring compliance with all regulations.
Maintain and update facility policies and procedures related to MDS and care planning.
Complete required medical forms, reports, evaluations, and documentation accurately and timely.
Create and manage a quality assurance program for assessments and care plans.
Collaborate with the Discharge Planning Coordinator on discharge-related care planning.
Participate in facility surveys and government inspections.
Monitor QI (Quality Indicators) and QM (Quality Measures), implementing improvements as needed.
Ensure timely completion and electronic submission of all MDS assessments.
Coordinate interdisciplinary team meetings for care planning.
Work closely with residents’ families and representatives to maintain collaborative care planning.
Support therapy services and assist with activity planning for residents.
Evaluate residents’ strengths and limitations to ensure individualized care and therapy interventions.

Requirements:

Current RN license in the state of Florida.
Strong team collaboration and communication skills.
Professional and empathetic demeanor with residents, families, and staff.
High level of accountability, initiative, and problem-solving skills.
Minimum of 2 years of experience in a long-term care or nursing home setting preferred.
