A Nurse Navigator collaborates within an interdisciplinary team to deliver specialized care focused on wellness, prevention, illness, and rehabilitation. Acts as an advocate for patients and their families, serving as a connection to resources and services. Provides guidance and expertise as a clinical resource and consultant to the healthcare team and school health staff. Contributes to the assessment and enhancement of patient care, specialty services, and programs through quality improvement initiatives. This position offers a Hybrid and flexible work environment that blends clinical experience with school experience working within the community.

Responsibilities:

Coordinates with the interdisciplinary team to guide patients and families through diagnostic and treatment options, providing emotional support, specialized care, and education.
Acts as a care coordinator and patient advocate, delivering comprehensive nursing care across all phases of the healthcare process.
Offers expert knowledge in specialty areas like school health, while addressing age-specific and cultural needs.
Integrates research and technology into practice and contributes to quality improvement initiatives.
Promotes health, disease prevention, and wellness education, serving as a clinical resource and supporting staff development.
Plans and implements community health programs with a focus on specialty care.
RN with Bachelor's Degree preferred.
Three to five years of current clinical experience required.
Previous School Nursing experience preferred
Must be self-directed and possess strong leadership and communication skills.
Demonstrated knowledge and clinical competence in the area of specialty with an emphasis on wellness and disease prevention throughout the life continuum.
Demonstrates ability for high degree of analysis, critical thinking and independent clinical judgment.

Licensure, Certifications, and Clearances:

Registered Nurse currently licensed in the practicing state*.
Current certification in specialty or obtained within one year
CPR required based on AHA standards that include both a didactic and skills demonstration component within 30 days of hire
Basic Life Support (BLS) OR Cardiopulmonary Resuscitation (CPR)
Act 33 / 34 / 73 with renewal
Current licensure either in the state where the facility is located or, if the facility is in a state covered by the multistate Nursing Licensure Compact (NLC) agreement, a multistate license issued by a participating NLC state. Hires and current employees working on an out-of-state NLC license who later change their residency to the state where the facility is also located will have 60 days upon changing their residency to apply for licensure within that state.
