Requirements:

- Graduate of an accredited school of nursing.

- One to two years of recent acute care experience in an institutional setting.

- Current licensure in the State of Arizona.

- BLS hands-on CPR certification.

- Valid driver’s license and auto insurance.

- Associate degree with one year of home health care experience preferred.

Responsibilities:

- Initiate and revise the plan of care based on patient status and needs.

- Develop a comprehensive care plan with patient and family involvement, focusing on palliative nursing actions.

- Provide counseling to patients and families to meet nursing and related needs.

- Deliver health care instructions tailored to each patient's assessment and plan.
