Job Objective:
Responsible for coordinating and tracking quality events within the assigned units/departments, abstracts clinical cases and provides appropriate follow-up with clinicians, leadership and physicians. Collaborates with multi-disciplinary teams to promote quality patient care and safety, improved performance, resolve issues and improve quality outcomes. Provides education to staff based on data from quality metrics.

Essential Responsibilities:

Demonstrates compliance with Code of Conduct and compliance policies, and takes action to resolve compliance questions or concerns and report suspected violations.

Follows up and investigates radiology discrepancies and follow up recommendations, as appropriate.

Follows up and intervention of positive culture reports and maintains logs, as appropriate.

Creates, maintains and presents quality metrics related to patient interventions and incident findings; educates staff and leadership on quality opportunities based on those metrics and findings.

Coordinates and follows up on customer service complaints in collaboration with leadership.

Acts as liaison between the Quality Department and assigned units/departments.

Facilitates and provides follow up information on Incident Reports from assigned units/departments, as needed.

Maintains positive interpersonal relations with peers, patients, families and physicians.

Actively participates and follows “Call Back Policy".

Collaborates with medical staff, case management, nursing, patient relations, and other departments as needed to maintain continuity of patient care, resolve issues, and promote quality patient care, as appropriate.

Collects and prepares data for quality review, including clinical indicators, patient satisfaction indicators and standards of care indicators, and presents data to the department on a monthly basis, as appropriate.

Serves as a resource and liaison for physicians and residents to provide education on patient related matters and ensures quality and quality outcomes are met, as appropriate.

Performs patient call backs to address inquiries and clinical issues, as appropriate.

Provides clinical education and resources to patients for post hospital care, as appropriate.

Assists in high risk case patient identification and collaborates findings with leadership, providers and/or other disciplines, as appropriate.

Participates daily to provide information and guidance to a hospital-wide multidisciplinary team conference regarding daily hospitalist statistics and barriers to discharge.

Interprets laboratory and test results and communicates abnormal findings to attending physician as needed.

Assists providers with daily administrative tasks including medication authorizations and completion of forms, as appropriate.

Educates providers on community availability of health care services including availability of prescribed medication and changes or updates in medication availability, as appropriate.
