As we continue to expand our team, we are seeking a skilled and dedicated Registered Nurse (RN) to join us in providing high-quality home care services.

Position Summary: We are currently looking for a Home Care Registered Nurse to join our team of healthcare professionals in Columbia County, New York. The ideal candidate will have a passion for delivering patient-centered care and be committed to improving the well-being of individuals in the community.

Responsibilities:

Conduct comprehensive assessments of patients' health status and develop personalized care plans.
Administer medications, perform wound care, and provide other skilled nursing services in accordance with physician orders.
Collaborate with other healthcare professionals, including physicians, therapists, and social workers, to ensure coordinated and effective care.
Educate patients and their families on managing health conditions and promoting a healthy lifestyle.
Monitor and evaluate patients' progress, adjusting care plans as necessary.
Maintain accurate and up-to-date patient records in accordance with regulatory standards.
Provide emotional support and compassionate care to patients and their families.

Qualifications:

Current and valid RN license in New York State.
Minimum of 1 year of nursing experience, with a preference for home care or community health experience.
Strong clinical and assessment skills.
Excellent communication and interpersonal skills.
Ability to work independently and as part of a collaborative team.
CPR certification.
Reliable transportation and a valid driver's license.
