Job Summary:

As a Registered Nurse in home care, you will be responsible for assessing patients' health needs, developing personalized care plans, and delivering high-quality healthcare services in the patients' homes. You will work closely with patients and their families to ensure their physical, emotional, and psychological well-being while promoting independence and improving quality of life.

Responsibilities:

Conduct initial assessments of patients' health status and needs.
Develop comprehensive care plans tailored to each patient's specific requirements and goals.
Monitor and evaluate patients' progress regularly, adjusting care plans as needed.
Provide skilled nursing care such as medication management.
Collaborate with other healthcare professionals, including physicians, therapists, and social workers, to ensure coordinated care.
Document patient care activities accurately and timely in accordance with regulatory requirements and agency policies.
Provide emotional support and counseling to patients and their families, addressing their concerns and facilitating coping mechanisms.
Ensure compliance with safety protocols and infection control measures to maintain a safe environment for patients and caregivers.
Participate in ongoing training and professional development activities to stay abreast of advances in nursing practice and home care standards.
Advocate for patients' rights and promote their autonomy and dignity in decision-making processes.

Qualifications:

Valid RN license in the state of Virginia.
Minimum of 2 years of nursing experience, preferably in a home care or community health setting.
Excellent clinical assessment and critical thinking skills.
Strong communication and interpersonal skills.
Ability to work independently and as part of a multidisciplinary team.
Compassionate and patient-centered approach to care.
Proficiency in using electronic health record (EHR) systems and other relevant healthcare technologies.
Reliable transportation and willingness to travel to patients' homes.
CPR certification (may be required).
Bachelor of Science in Nursing (BSN) preferred.

Working Conditions:

Flexible scheduling, including evenings and weekends, to accommodate patients' needs.
Exposure to various home environments, which may include limited space, pets, and other household factors.
Physical demands such as lifting and transferring patients, bending, and prolonged standing.
