Requirements:

AZ DRIVERS LICENSE
FAMILY ATMOSPHERE
PROOF OF CAR INSURANCE (if wanting to run errands/doctor appts)
FINGERPRINT CLEARANCE CARD
CURRENT TB TEST RESULTS
CPR CERTIFICATION (will accept on-line training)
1ST AID CERTIFICATION (will accept on-line training)
6 MONTHS EXPERIENCE PREFERRED
3 PROFESSIONAL REFERENCES
