Position Overview: The RN Hospice Manager is responsible for providing leadership support for all aspects of client care within the Hospice department. Leadership responsibilities include operational management, process improvement, and workforce oversight. The RN Manager applies evidence-based practices to provide ongoing evaluation and refinement of Hospice Services. This role develops and maintains positive relationships and interactions with collaborative agencies, departments, and services.
Essential Duties & Responsibilities:
- Implements process controls to prevent errors in documentation systems, policies, and procedures.
- Directly supervises the interdisciplinary team including consultants and partners in the provision of services.
- Responsibilities include interviewing, hiring, training and terminating (with administrative approval) all interdisciplinary team members; planning, assigning, and directing work; appraising performance; rewarding and performance management of colleagues; addressing complaints and resolving problems.
- Implements current requirements of the Hospice Conditions of Participation for Medicare, applicable accreditation standards, and state or other rules and regulations as applicable to the program.
- Attends/Leads Interdisciplinary Team (IDT) meetings, ensuring core members identify and incorporate appropriate actions for clients, document necessary components of service delivery, and all re-certifications are completed as required.
- Patient care needs are identified and discussed with the full complement of the IDT.
- Monitors and audits documentation of the patient-centered plan of care and the implementation of interventions for patient care including changes in level of care, medication changes, changes to visit frequency, or need for emergency transportation.
- Leads the update and development of patient care policies and emergency procedures.
- Ensures hospice services are available 24/7.
- Follows state regulatory requirements for supervision of nursing services.
- Manages all members of the IDT including aspects of scheduling, productivity, mentoring, pay practices, timekeeping, performance reviews, and colleague support.
- Models desired behaviors including timeliness, punctuality, attendance, collaboration, open communication, and equitable treatment.
- Actively collaborates regarding QAPI activities and provides feedback and input into Performance Improvement.
- Participates in performance improvement programs.
- Analyzes and reports on quality metrics including Hospice Item Set, CAHPS, and symptom management outcomes.
- Models and facilitates communication between colleagues including medical records, business office, intake, clinical team, and leadership.
- Participates in care delivery for patients as needed including conducting admissions, recertification visits, routine visits, and discharge visits.
- Participates in after-hours shifts as needed including primary and back-up on-call.
- Directs responsibility for the management of Pharmacy, Medical Supplies, Durable Medical Equipment, mileage, and labor for program to meet budget expectations.
- Ability to prioritize time and changes in tasks in a fast-paced environment and to effectively communicate with and deal with concerns from residents, families, the general public, volunteers, staff, and managers.

- HCCOOR- Office Coordinator Hospice
- HCRN- RN Home Care-Hospice
- HCHUC- HUC Home Care-Hospice
- SSSWKR- Social Worker
- SBVCOR- Spiritual Bereavement Coordinator
Skills and Experience:
Required:
- Registered Nurse MN/WI
- Minimum three years in Healthcare, Preferably in a Nursing Facility, Home Health, or Hospice
- Current valid driver's license and reliable transportation
- AHA/CPR Certification
- Experience with OASIS/MDS requirements and documentation
Preferred:
- Bachelor degree
- Experience in leadership and management
- Strong verbal and written communication skills
- Intermediate computer skills Word, Excel, PowerPoint, and patient information databases (Cerner, MatrixCare).
