Per Diem (HHA or CNA)

This is a Per Diem Job you have to be able to work all the shifts that we give you no matter where it is and you have to be able to work at anytime during the day or night. If you have to travel past 25 miles we will give you a travel bonus.

Synergy HomeCare is seeking compassionate and dedicated Home Health Aides (HHA) and Certified Nursing Assistants (CNA) to provide high-quality, non-medical home care services to clients in their homes on a per diem basis. As a per diem caregiver, you will have the flexibility to choose shifts that best fit your schedule while making a meaningful impact on the lives of those in need of assistance.

Key Responsibilities:

Provide companionship and emotional support to clients.
Assist clients with activities of daily living (ADLs), including bathing, dressing, grooming, and toileting.
Support mobility, including transferring and ambulation assistance.
Prepare light meals and assist with feeding as needed.
Perform light housekeeping duties, such as laundry, vacuuming, and tidying up.
Monitor and report changes in clients' physical, emotional, and mental conditions to supervisors.
Assist with medication reminders as directed by the care plan.
Follow all safety and infection control guidelines.
Maintain accurate and timely documentation of care provided.
Provide respite care for family caregivers as needed.

Qualifications & Requirements:

Valid HHA or CNA certification in [Specify State].
Previous experience in home care, assisted living, or similar settings preferred.
Ability to work independently and provide compassionate care.
Need to have a car to have the ability to travel to clients' homes.
Excellent communication and interpersonal skills.
CPR and First Aid certification (preferred but not required).
