We’re looking for a Registered Nurse (RN) Case Manager to join our home health team. If you thrive on providing high-quality care, coordinating services, and making a difference every day, we’d love to meet you.

Competitive Pay: $38 - $45 per hour (based on experience)
Four Weeks Paid Time Off plus six paid holidays
Mileage Reimbursement at $0.50 per mile
Flexible Schedules – Day, evening, and weekend shifts available

On-Call Stipend – $150 per week when assigned as the on-call RN

Your Role

Perform patient assessments and develop care plans
Complete OASIS assessments at key time points
Coordinate services and ensure seamless patient care
Maintain accurate documentation and progress notes
Serve as a trusted support for patients and their families

What You Need to Succeed

Active Connecticut RN License
CPR Certification
Home Health Experience Preferred
Reliable Transportation (Valid Driver’s License & Insurance)
Strong Communication and Organizational Skills
