Department: Home Health & Hospice

Definition of Position:

Provides direct oversight of the clinical care staff including RN, LPN’s, CNAs, and RN Coordinators, PTA’s, OT and SLP, and Coordinates all clinical aspects of patient care for the Home Health and Hospice department. Provides direct supervision of RNC’s , RN's, LPN's, CHHA's, CNA’s, Care Givers and Therapies. Works collaboratively with the Manager in planning, organizing, directing, supervising, and evaluating all departments on a continuing basis. Helps develop and maintain all home-based community programs and needs. Will work closely with clinical staff in maintaining community facility relationships to ensure quality care and continuity of care. Actively participates in performance improvement, staff scheduling and timecard approval and evaluations. Assumes duties of the Nurse Manager during absences. Provides direct and indirect patient care as outlined in Registered Nurse and RN Coordinator job description. Performs other duties as assigned.

Participates in the Performance Improvement process. Maintains quality and efficiency standards as set by supervisor and makes recommendations for improvements.
Participate in activities that promote professional growth and self-development. Attend pertinent in-services, departmental and other meetings as requested by supervisor.
Adheres to GSHCS standards for timeliness and dependability.

Maintains an in-depth knowledge of principles, practices, standards, and techniques and demonstrates knowledge in accordance with GSHCS policies and procedures.

Maintains and demonstrates a thorough knowledge of departmental safety policies and procedures as they pertain to the job, including the fire and disaster plans.
Maintains a safe environment for patients, guests, and staff.
Maintains confidentially Protected Health Information and other department information.

Must be able to supervisor others and plan, organize and implement activities. Maintains harmonious and cooperative relations with fellow employees, physicians, patients, other customers and business contacts.
Supports GSHCS’s vision, values, success factors, and interaction agreements.

Represents Home Health department in working as part of GSHCS team.
Keeps management, colleagues & stakeholders informed about work status, problems, and issues.
Represents GSHCS with business partners & customers.

Qualifications

Education: Current RN License, BSN required within five years of employment

Registration/licensure: Oregon State license required, current BLS. Valid Oregon driver’s license and proof of current auto insurance.

Experience: Minimum of 5 years’ experience as an RN. Previous health care management preferred.

Home Health experience desired.

Job Knowledge: Working knowledge of current Medicare, Medicaid and insurance regulations. Requires knowledge of medical terminology.

Physical Requirements: Must be able to bend, stoop, reach and occasionally lift 50 pounds or more with frequent lifting, carrying, pushing and pulling of objects weighing up to 25 pounds. Frequent sitting and/or standing/walking throughout the day required. Pushes/pulls or moves/lifts heavy equipment/supplies. Must be able to identify, match and distinguish colors.

Working Conditions:

Inside and outside environment, not always protected from the weather temperature changes. Inside environmental changes are mainly those inside patient homes that may include temperature variations, smoke, unclean homes, various sizes, and types of pets and structural faults/barriers. Subject to frequent exposure to infection, contagious disease, and hazardous relationships. Variable noise levels. Also subject to rapid pace, multiple stimuli, unpredictable environment, and critical situations.

40 Hours USD $103,640.16/Yr. USD $166,121.28/Yr. 01-3100 Hospice First Shift
