About the Role:

As a Licensed Practical Nurse atColumbine Heath Services, you willprovide direct resident care utilizing the nursing process. Delivers care within the scope of practical nursing, in compliance with recognized nursing techniques, federal and state requirements, established nursing standards, and company policies and procedures. Assumes personal accountability for the company’s core mission and values.

What you will be doing as a LPN:

Executes the patient-centered care plan as directed by the client’s physician and the designated RN Case Manager (e.g., medication management, wound care, etc.).
Reports client condition changes to the RN Case Manager or Clinical Manager and adheres to the agency’s emergent status change reporting protocol.
Collaborates with the interdisciplinary care team to achieve patient-centered goals and promotes holistic care by reinforcing principles taught by other teammates.
Delivers timely and accurate documentation surrounding the client’s treatment activities.
Contributes to the agency’s strategic vision, including delivering first-class patient-centered care, engaging in effective team collaboration, and supporting financially responsible operations.

What We're Looking For:

Experience and Education:

Must possess a nursing degree from an accredited university.
Clear, unencumbered licenses form the State of Colorado as a Licensed Practical Nurse.
6 months- one year of related experience or training.
Preferred experience in the geriatric population in a long-term care facility, assisted living, or other related heath care facility.

Skills:

Ability to effectively able to read and interpret documents.
Ability to communicate effectively.
Basic mathematical skills.
Ability to reason and problem solve.
Ability to lead and supervise nursing assistants for the assigned unit.
Basic technical skills.

Our Total Rewards package includes a salary range of between $27 - $32/hr. DOE
