TITLE: LICENSED VOCATIONAL NURSE
Rate: $37/hr
STATUS: P/T 4 day/week
Shift: 3pm-11:30pm

Golden Ventura CRT Services include crisis, transitional and long term residential treatment, day rehabilitation, assisted and independent living, Mental Health Rehabilitation Center, vocational rehabilitation and employment services.
DEFINITION:Under the supervision of the Program Director, participate in the operation of a 15-bed Crisis Residential Treatment Facility for the stabilization, support and treatment for people who have mental/emotional disabilities.The program is for psychiatrically acute or sub-acute adults who require 24-hour supervision and intense structure.The program’s primary goal is to divert clients from hospitalization or act as an alternative to hospitalization and to re-integrate clients into the community.This is a non-exempt position.
DUTIES AND RESPONSIBILITIES:
1.Provide daily nursing services such as quality nursing care, individual counseling, medication support, educational programs, and case management services for clients.
2.Participate in updating of treatment plans, administer medications, and monitor interactions, side effects, etc., in consultation with Program Director, physicians, psychiatrists, and all other treatment staff.
3.Chart and report any significant changes in condition and general progress of residents to Program Director, physicians and psychiatrists.
4.Monitor all medications and sign and certify the accuracy of counts and inventory each day.
5.Follow and adhere to all medication policies regarding dispensing, administration, record keeping, storage and accountability for inventory.
6.Ensure that preparation of doses for more than one scheduled administration time shall not be permitted.
7.Ensure all medication is administered by licensed medical or licensed nursing personnel.
8.Ensure medications are administered as soon as possible, but no more than two hours after doses are prepared.
9.Report any sudden marked changes or adverse changes in signs, symptoms, or behaviors; unusual occurrences involving clients; change in weight of 5 pounds or more within a 30 day period; any untoward response or reaction by a client; errors in administration of medications that is life threatening or presents a risk to a client; facility’s inability to obtain or administer promptly and timely, any drugs, equipment, supplies or services.All attempts to notify physician(s) shall be noted in the client's record (time, method, and name of the person acknowledging contact, and if physician is not available, the arrangements for emergency medical care).No medication or treatment shall be administered except on the order of a person lawfully authorized to give such order; medication shall be administered as prescribed by physicians; tests and taking of vital signs, upon which administration of medication or treatment are conditioned, shall be performed as required and the results recorded; all nursing staff will be trained to do the same.
10.Facilitate an integrated, cohesive and supportive treatment approach and attitude.Act as a role model and participate as a member of a multi-disciplinary treatment team with other staff.
11.Assist residents in keeping their daily activity schedules in accordance with individual treatment plans.
12.Possess a clinical understanding of and therapeutic relationship with clients to facilitate an integrated, cohesive, coordinated and supportive treatment approach.
13.Assist in the admission process and preparation of all required forms on clients accepted into the program, including initial intakes and treatment plans, in an accurate and timely manner.
14.Complete accurate and timely charting in accordance with Quality Assurance Standards and Medi-Cal/ Medicare guidelines and requirements.
15.Participate in daily individual counseling to residents, medication support, education programs, and case management services for clients.
16.Coordinate with all persons involved in residents' treatment to include but not be limited to: family, case managers, treatment coordinators, therapists, etc., to assure continuity of resident care.
17.Provide supervision of residents, have a working knowledge of, and provide crisis prevention and intervention for residents.
18.Protect consumer rights and maintain residents' confidentiality.Assure residents' safety at all times.
19.Work the hours required and as scheduled by the Program Director, including overnights, weekends, holidays, occasional fill-in for other staff, and on-call responsibilities.Contribute to the ongoing maintenance, safety, and housekeeping of the facility.
20.Ensure that all employees wear name and title identification badges while on the premises.
21.Participate in in-service trainings. Complete training in the emergency procedures. Attend All Staff Meetings.
22.Complete twenty (20) hours of continuing education, within each twelve month period following the assumption of duties.
23.Provide for the care and safety of residents without physical or verbal abuse, exploitation or prejudice.Report any incidents or suspected incidents of abuse according to current law.
24.Report observations or evidence of violations of any personal rights specified in Titles 17 and 22, and any unusual or special incidents as defined by Title 17 and 22.
25.Perform all other duties and responsibilities as assigned by supervisor.
26.Must obey all federal, state, and local laws and GSHC, Inc. Policies and Procedures.
27.Comply with the GSHC policy requirement to submit proof of valid CDL, maintain good driving record, and submit proof of auto insurance and must meet insurability requirements of insurance carrier.
28.Requires driving own vehicle to transport consumer(s) when program vehicle is unavailable.

QUALIFICATIONS:
At least 18 years of age. Provide Health Screening with negative TB results. Have Criminal Record Clearance. Current CPR/First Aid Certificate.
LICENSE:Possession of a valid California license to practice as a Licensed Vocational Nurse, issued by the California Board of Psychiatric Technician Examiners, current and in good standing.
EXPERIENCE:Requires one year experience in mental health field.Bi-Cultural/ Bi-Lingual (Spanish) experience preferred.Understanding, observation and recognition of symptoms and reactions requiring knowledge of cause and effect factors in dealing with care of mental health clients.Basic computer skills for using word processing (WORD), email (OUTLOOK), Internet, and related business applications.
SKILLS AND ABILITIES:Skills and abilities to handle clinical issues in a fast paced environment resulting from the complexity of mental health treatment facility issues.Ability to work with a variety of people in situations varying from community, hospital, family and other sources.Ability to communicate well in English, both verbally and in writing.Ability to communicate verbally by phone in English in a clear and concise manner.Basic computer skills for using word processing (WORD), email (OUTLOOK), Internet, and related business applications.
PHYSICAL ABILITIES:Requires performance of facility chores, errands and ability to lift 40 pounds approximately 3-4 times per week.Navigate several flights of stairs during each work period.Ability to work all shifts for a 24-hour, 7-day a week facility.Ability to drive safely in heavy traffic and inclement weather.
DRIVERS LICENSE REQUIRED:A valid California Driver’s License - Class III/C.Proof of auto insurance.
