Primary Care

Catholic Health has over 60 PCPs situated at over 20 primary care practices across Long Island. The primary care physicians of Catholic Health Physician Partners are committed to understanding all your needs in order to develop a personalized health plan that is right for you. If you ever need more specialized care, your Catholic Health primary care provider can easily refer you to, and collaborate with, one of our many award-winning specialists across our integrated health care system.

1. Conducts comprehensive or episodic health history and physical assessment of patients/families with acute, chronic and/or potential health problems.

2. Collaborates with Medical Provider to establish medical diagnoses and treatment plans

3. Provides for continuity of care between clinic visits

4. Assigns specific orders within the limits of the practice agreement/scope of practice

7. Provides appropriate direction to clinic staff regarding patient care issues.

8. Participates in a team model of care delivery to identified group/panel of patients.

9. Meets regularly with appropriate physicians; works collaboratively to effect programmatic changes and develops protocols.

11. Participates in quality assessment and improvement activities. Identify important aspects of care and assure findings shared to improve patient experience and practice operations.

12. Works in collaboration with staff to provide on-going patient care and management

17. Holds responsibility for timely and accurate written documentation or patient care encounters/procedures.

Requirements:

Primary Care Practice Setting experience preferred
Masters in an AMA approved NP program - FNP
NYS Nurse Practitioner License
NP Board Certification - FNP
BLS and ACLS certification

Salary Range

USD $66.55 - USD $90.00 /Hr.
