Job Summary
The Nurse Coordinator for the Heart Failure Disease Management portion of the Advanced Heart Failure comprehensive Care Center (AHF CCC) coordinates care for our patients being evaluated for general heart failure management, as well as those who may be moved into advanced heart failure therapies workups. This role will coordinate between inpatient and outpatient departments, and manage patient panels including recommending diagnostic testing, lab testing, and other care therapies. This role regularly educates patients on heart failure disease, treatment, therapies, medication adherence, and more. This role will interface between multiple UCSF departments, review incoming referrals, and coordinate outpatient care for patients with heart failure. The Nurse Coordinator will also rotate with other Nurse coordinators in the AHF CCC to work in the AHF CCC Ambulatory IV diuretic clinic to assist the AHF CCC APP in care and monitoring of patients being seen and treated for acute heart failure requiring intravenous diuretics.
The final salary and offer components are subject to additional approvals based on UC policy.
Your placement within the salary range is dependent on a number of factors including your work experience and internal equity within this position classification at UCSF. For positions that are represented by a labor union, placement within the salary range will be guided by the rules in the collective bargaining agreement.
The salary range for this position is $91.20 - $118.34 (Hourly Rate).

Required Qualifications
- Bachelor’s degree in nursing
- Five years’ experience in Cardiovascular nursing including heart failure disease management, heart and/or lung transplant, mechanical circulatory support, pulmonary hypertension, cardiology, or other related cardiovascular area
- Knowledge of guideline directed medical therapy for the treatment of heart failure
- The ability to work independently and autonomously within scope of licensure
- Strong MS Office computer skills (word, excel, power point), databases
- Excellent organizational skills, proficiency in meeting deadlines, ability to work independently, prioritize workload and manage numerous concurrent tasks in high stress environment
- Previous experience with development of policies, procedures, implementation of process improvement workflows
- Demonstrated excellent written and verbal communication skills
- The flexibility to orient and work at all UCSF Medical Center location
Preferred Qualifications
- Critical Care Cardiology, Advanced Heart Failure/Pulmonary Hypertension experience
- Ambulatory nurse coordinator experience 2 years or more
- Apex SuperUser
- Phoenix Experience
- Master’s degree in nursing
- Working knowledge in assessing advanced heart failure/cardiac and/or pulmonary hypertension
- Experience with managing and adjusting heart failure medications
- Prior experience with professional scholarly nursing presentation(s)
License/Certification
- Active Registered Nurse licensure in the State of California
- Active American Heart Association CPR Certification
