Example Job Duties/Expectations

Medication and treatment administration.
Checking residents per direct care staff concerns and post incident.
Completing documentation – i.e. 24-hour report, nursing notes, medication administration record and physician orders.
Acts as a liaison with physicians by communicating medical concerns of residents with physicians and other medical consultants and then informing team members of treatment recommendations.
Attends meetings i.e. medication reviews, admission/discharge and special team meetings.
Assists with physical exams of individuals.
Full-time position on 2nd shift – 2:20PM to 10:50PM with ½ hour unpaid meal break
60 cent shift differential for any hours worked on 2nd or 3rd shifts
Every other weekend off
Mandatory hours are required

License to practice nursing as a licensed practical nurse issued by Ohio Board of Nursing pursuant to Section 4723 Ohio Revised Code & proof of completion of an approved course in medication administration.

Note: Licensed Practical Nurses who administer dialysis treatment must possess Certification in IV Therapy pursuant to Section 4723.17 Ohio Revised Code.

TDC is a residential facility located in Northwest Ohio. We support youth aged 10-17 and adults with developmental disabilities. Our direct care staff and nurses provide 24/7 care to the individuals who call TDC home. We focus on support and stabilization so our individuals can integrate into an environment that best meets their wants and needs.
