SUMMARY

Provides assistance in Resident care planning activities, including the MDS and MMQ processes. Participates in timely completion of MDS process (Minimum Data Set, discharge and re-entry tracking forms).

ESSENTIAL DUTIES AND RESPONSIBILITIES include the following. Other duties may be assigned.

·Assists the Resident Assessment Coordinator in fulfilling responsibilities, as directed.

·Completes assessments of Residents that directs the plan of care.

·Conducts orientation sessions on the MDS process to all employees involved.

·May coordinate information systems operations and education for the clinical department.

·Responsible for contacting Medicare benefits hotline as required per company guidelines.

·Develops schedule of due dates for periodic assessments.

·In the absence of the Resident Assessment Coordinator, may perform duties of that position with oversight by a Registered Nurse (if applicable).

·Completes required forms and documents in accordance with company policy and state and/or federal regulations.

ENTRY QUALIFICATIONS

·Graduate of an accredited school of Nursing.

·Valid State registration as an LPN/LVN

·Knowledgeable of OBRA, Perspective Payment System, and MDS process.

Starting Rate of $38.00 Negotibale upon experience.
