GAFC Registered Nurse (RN)

We are looking for a dedicated Registered Nurse (RN) to join our Group Adult Foster Care (GAFC) program. The ideal candidate will be passionate about providing high-quality care to elders and adults with disabilities.

Qualifications

Must be fully licensed by the Massachusetts Board of Registration in Nursing.
Must have at least two years of recent experience in direct care for elders or adults with disabilities.
Strong clinical assessment and care planning skills.
Ability to train, evaluate, and supervise direct care aides.

Responsibilities

The GAFC RN will be responsible for:

Completing and coordinating all applicable clinical assessments and evaluations.
Developing and continuously reviewing each member’s GAFC plan of care.
Selecting, training, evaluating, and supervising GAFC direct care aides in collaboration with the care manager.
Reviewing the PCP Summary Form.
Monitoring the health status and treatment plans of all members to ensure proper care delivery.
Reporting any changes in health status to the member’s PCP.
Reviewing GAFC direct care aide logs every 30 days.
Conducting on-site visits with each member in compliance with 130 CMR 408.505(B)(6).
Completing nursing progress notes for each visit or as needed based on the member’s condition.
Submitting semi-annual health-status reports to the member’s PCP.
Planning and implementing discharges and transitions from the GAFC program.
Conducting orientation for each GAFC direct care aide before they begin providing personal care.
Providing ongoing training to GAFC direct care aides on health and aging.
