Candidate would thrive in a fast paced environment and be team oriented. Provider will need to work collaboratively with physician to coordinate and deliver the highest quality of patient care. Candidate will also have a desire to develop strong ties with patients and the community in which they serve.
WHAT YOU CAN EXPECT:
Formulate medical diagnosis based on patient admitting status, and physical findings. Implement plan of care.
Communicate to Physicians, their assessment through surveillance exams, injuries, treatment and procedures as deemed necessary.
Develop individualized plan of care for all assigned patients. Consistently evaluate the effectiveness of care provided to the patient.
Perform interventions to meet holistic needs of patient, under the direction of the Medical Director, and under the leadership of the Client Resource Manager.
