JOB OVERVIEW
The Diabetes Educator RN is responsible for instruction of patients in diabetes self-management, facilitating patient education programs and developing patient teaching tools. This position accomplishes this by assessing, planning and implementing educational programs to meet the needs of patients, clinical staff, and the organization. The Educator ensures practices are aligned with the Franciscan Alliance values and the strategic goals of the department and organization to improve patient and staff outcomes.
WHAT YOU CAN EXPECT
Change Management Acts as a change agent by introducing and supporting new ideas as well as assisting in the implementation and sustainment of change strategies.
Communication Facilitates the communication process between management, staff, ancillary departments, and professional contacts across the Franciscan Alliance. Utilizes appropriate channels of communication.
Leadership & Collegiality Serves as a leader in promoting the professional growth of peers. Facilitates interdepartmental collaboration to achieve project and organizational goals. Serves as a resource for and contributes to the professional development of interdisciplinary team members.
Patient Education Educates patients about their disease, prescribed treatment, and side effects of treatment and how to manage them.
Self-Care Empowers patients, families, and caregivers to assume as much responsibility for their care as possible, within the constraints of age, illness, and other factors.
Education Development & Coordination Assesses staff training or education needs by collecting data and feedback. Plans educational interventions based on collected data. Facilitates the development of educational activities based on the target audience learner needs ensuring adult learning principles are incorporated. Coordinates or implements the training or development.
QUALIFICATIONS
Bachelor's Degree required, Master's Degree preferred
Certified Diabetes Educator (CDE) - Required
One year of experience with diabetes patients preferred.
