Day Shift - 7.5 Hours (United States of America)

The Nurse Practitioner (NP) will provide Pediatrics & Developmental Behavioral Pediatrics services to pediatric patients and their families in our outpatient Family Health Centers, which operate under a patient-centered medical home (PCMH) model. This role involves conducting assessments, implementing treatment plans, and coordinating care with an interdisciplinary team to support pediatric care, early childhood development and mental health. The NP will play a key role in integrating developmental and behavioral screening of children 0 to 3 into primary care through Healthy Steps initiative in collaboration with Institute for Basic Research Baby Steps quality improvement project, ensuring best practices, and optimizing care coordination.

Essential Duties and Responsibilities:

-Provide comprehensive behavioral health evaluations, treatment, and follow-up care for pediatric patients.

-Collaborate with primary care providers, pediatric specialists, rehabilitation services, research investigators, nursing staff, health educators, social workers, and behavioral health providers to deliver integrated care.

-Implement workflows, policies, procedures, and best practices to enhance service integration and care coordination.

-Conduct early childhood developmental and mental health screenings and provide appropriate interventions.

-Develop and implement individualized treatment plans in coordination with the interdisciplinary team.

-Provide patient and family education on behavioral health conditions, developmental milestones, and available community resources.

-Ensure timely and accurate documentation of patient interactions and treatment plans in the electronic medical record (EMR) in compliance with regulatory requirements.

-Participate in case conferences, team meetings, and quality improvement initiatives to enhance patient care.

-Stay up to date on evidence-based practices in pediatric behavioral health and contribute to program development.

Required Qualifications:

Experience in behavioral health settings or pediatric behavioral health required.

Experience in early childhood development, infant and early childhood mental health, or developmental behavioral pediatrics preferred.

Certification/Licensure:

NYS Professional Registered Nurse License required.

NYS Nurse Practitioner Certification required (Family or Behavioral Health preferred).

Education:

Nurse Practitioner: MSN required (Family Practice or Behavioral Health preferred).

Technical Skills:

Proficiency in electronic medical records (EMR) systems required.

Knowledge of Microsoft Office applications required.

Experience with patient registries and data mining applications preferred.

Physical and Environmental Demands:

Work is primarily performed in an outpatient clinic setting.

May require prolonged periods of sitting, standing, or walking.

Requires ability to use a computer for documentation and communication.

Full Time Hours, various hours to be discussed at interview

Salary Range: $135,000 - $140,000
