Novant Health is hiring a home health registered nurse for the weekendshift for our New Hanover Regional Medical Center location in Wilmington, NC.

We’re seeking a registered nurse to join our team in our home health department. We are a very supportive team and always available to help each other out. In this position, you will be responsible for coordinating home care services which includes establishing and maintaining the patient’s plan of care. This includes the quality, customer service and financial management of your assigned caseload. Caseload can cover up to 7 counties which include Brunswick, New Hanover, Columbus, Onslow, Sampson, Blayden and Pender. Administrative tasks include closing charts, reporting, calling physicians, general administrative responsibilities are completed remotely from home. Weekend RN also provides on-call support.

Schedule: Weekend, Friday 3 pm - Monday, 8 am. Monday administrative tasks are completed remotely from home
Department: Home Care – Skilled Nursing Unit
Responsibilites:
- Conducts admissions, recerts, ROC, and discharges for the assigned case load.
- Conducts routine nursing visits as needed.
- Assesses patient needs.
- Coordinates patient care between the patient, caregiver, physician, and other appropriate disciplines.
- Responsible for the completion of all home health documentation from home.
- On-call responsibility.

Qualifications

Graduate of an accredited school of nursing required.

Associate's or Bachelor's degree in nursing.

Current valid North Carolina nursing license required.

Basic Life Support (BLS) through American Red Cross or American Heart Association required.

Driver’s License Class C.

Minimum 1 year of home health or related experience required.
