What You’ll Do:

Coordinate and oversee ESRD patient care, including tracking lab values, vital signs, weight, and outcome management.
Ensure the highest level of care by prioritizing patient safety, comfort, and well-being.
Work with a dynamic team to deliver personalized care to patients with complex medical needs.
Build meaningful, long-term relationships with patients and their families in a close-knit outpatient setting.
Supervise a group of patients alongside PCTs to conduct patient observations, measure stats, and set up dialysis machines.
Deliver care with a combination of expertise, empathy, and hands-on involvement in a fast-paced environment.

What We’re Looking For:

A current Registered Nurse (RN) license in your state of practice.
At least 18 months of registered nursing experience, ideally with experience in ICU, CCU, ER, or Med-Surg settings.
Dialysis experience is a plus, but not required.
Ability to lead and supervise a clinical team with compassion and effectiveness.
Associates Degree in Nursing (A.D.N.) from an accredited nursing school (BSN preferred).
Certification as a Certified Nephrology Nurse (CNN) or Certified Dialysis Nurse (CDN) is preferred.
Basic computer skills and proficiency in MS Word and Outlook.
