AMBULATORY CARE: Join us at Ambulatory Care where we strive to provide comprehensive, cost-effective, compassionate health care for our diverse community. Ambulatory Care consists of a system of primary care and specialty care medical clinics throughout Ventura County. These clinics offer a medical home to meet the whole family's health needs. Patients receive routine checkups, immunizations, preventive care, treatment for acute and chronic conditions, and coordinate with specialists when needed. They provide care on an outpatient basis at all levels.
THE POSITION: Under supervision the Referral Center Licensed Vocational Nurse (LVN) is responsible for managing a caseload of individuals with specific referrals & gastrointestinal (GI) procedures. The LVN coordinates and facilitates the delivery of care by using screening tools and guidelines to assess and identify nursing care needs of each patient. They schedule specialty appointments in accordance with physicians' orders, follow up on referrals, and ensure appointments are timely. Additionally, the LVN handles a high volume of patient phone calls, routing them appropriately for prompt follow-up and care. The LVN also reviews patient charts using electronic systems such as Cerner, eReferral, and Microsoft Office to ensure accurate documentation and care coordination.
IDEAL CANDIDATE: The ideal candidate for this position is a compassionate and detail-oriented Licensed Vocational Nurse (LVN) with a strong understanding of patient care coordination, particularly managing diverse caseloads. The candidate should be skilled in using screening tools and clinical guidelines to assess and address nursing care needs, while demonstrating the ability to work efficiently and collaboratively as part of a team. Strong organizational skills, excellent communication, and interpersonal skills are essential. Experience with electronic health record systems, such as Cerner, eReferral, and Microsoft Office is a plus.

The County of Ventura offers an attractive compensation and benefits package, subject to eligibility requirements, that includes:
- EDUCATIONAL/BILINGUAL INCENTIVE: Incumbents may be eligible for an educational incentive of 2.5%, 3.5%, or 5% based on completion of an Associate's, Bachelor's, or Master's degree that is not required for the classification. Incumbents may also be eligible for bilingual incentive depending upon operational need and certification of skill. In order to qualify for this incentive, incumbents in eligible positions must take and pass the applicable bilingual fluency examination.
- CERTIFICATION PAY: $0.813 per hour based on scheduled work week hours for each qualified certification up to a maximum of five (5) certifications. One of the five certifications may be paid at $2.00 per hour for a National Certification.
- CONTINUOUS COUNTY SERVICE INCENTIVE PAY: 1% for 7 years, 1.25% for 12 years and 1.5% for 17 years
- FLEXIBLE CREDIT ALLOWANCE: Biweekly contribution towards medical, dental, and/or vision insurance from authorized plans.

Licensed Vocational Nurse is represented by the California Nurses' Association (CNA) and is eligible for overtime compensation. Salary placement will be determined according to the current California Nurses' Association (CNA) memorandum of agreement.
The eligible list established from this recruitment may be used to fill current and future Regular (including Temporary and Fixed-Term), Intermittent, and Extra-Help vacancies for this position only. There is currently one (1) Regular vacancy within Ambulatory Care Referral Center.
TENTATIVE SCHEDULE
OPENING DATE: January 6, 2025
CLOSING DATE: January 17, 2025, at 5:00 p.m. Duties may include but are not limited to the following:
- Performs patient care functions and initiates appropriate nursing interventions as outlined in standards of care and the individual patient care plan in support of professional staff;
- Manages a caseload of individuals with specialty referrals and GI procedures by coordinating and facilitating delivery of care; uses screening tools and guidelines to inform, implement, and monitor plan of care for assigned patients; maintains health boundaries and professional ethics;
- Refers individuals and families to available community medical clinics, behavioral health, public benefits; follows-up on referrals or appointments scheduled;
- Ensures appropriate documentation in the electronic medical record;
- Attends meetings and trainings as required;
- Demonstrates cultural sensitivity and competence with patients;
- Communicates with diverse patients regarding disease prevention, health maintenance, and physician instructions;
- Performs basic assessments (data collection & entry) and documents variances;
- Interprets physician's written orders, and takes necessary steps to ensure completion;
- Safeguards patient's health and safety by reporting abuse and neglect, maintaining patient confidentiality, and maintaining current skill and knowledge of safe and competent patient care;
- Utilizes programs such as Cerner, eReferral, and Microsoft Office to review patient charts, schedule referrals, and data entry;
- Handles high volume of patient phone calls; and
- Performs other related duties as assigned.

EDUCATION, TRAINING, and EXPERIENCE:
Requires one (1) year of full-time licensed vocational nursing experience in a health care setting that includes at least six (6) months of full-time licensed vocational nursing experience comparable to that obtained at the Ventura County Medical Center or Ambulatory Care in the Health Care Agency.
NECESSARY SPECIAL REQUIREMENTS:
- Possession of a current valid license as a Vocational Nurse issued by the State of California
- Bilingual in English/Spanish
DESIRED:
- Certifications (BLS, ACLS, PALS)
- Microsoft Office or similar experience
- California Driver License
KNOWLEDGE, SKILLS, AND ABILITIES:
Working to thorough knowledge of:
- the techniques and practices of nursing;
- medical terminology, routines, and equipment;
- the role in abuse/neglect reporting;
- confidentiality policies.
Skill in:
- the transcription of physician’s orders, including telephone/verbal orders and the completion of those orders; and
- general computer skills, understanding how to create, save, organize, and retrieve files on computer, as well as cloud storage;
- familiarity with Microsoft Office or similar tools, including email software;
Ability to:
- apply a variety of techniques, methods and procedures utilizing required equipment for patient care;
- interpret medications as ordered by physicians;
- follow agency and unit policies and instructions as given;
- establish and maintain the confidence of patients;
- maintain confidentiality of patient/client information;
- work effectively in different healthcare and/or hospital departments or shifts as required; establish and maintain effective interpersonal relations with other staff members, patients, and their families; and
- communicate effectively both orally and in writing.
