In this RN Care Manager role, will be an essential element of an Integrated Care Model by relaying the pertinent information about the member needs and advocating for the best possible care available, and ensuring they have the right services to meet their needs.
Expect to spend about 50% of your time in Southern Pines, Troy, Pinehurst, Richmond, Anson, Montgomery, Moore, Sanford, Rockingham, NC, visiting our members in their homes or in long-term care facilities. You’ll need to be flexible, adaptable and, above all, patient in all types of situations.
We offer our established staff the option to work 4 10-hour days (7 am-6 pm) in lieu of the traditional 8 hour 5/day week schedule. *after employee has demonstrated competency with the role and are able to meet metrics, etc.
Primary Responsibilities
Additionally, there is no “on-call” or weekend requirements.
- Assess, plan and implement care management interventions that are individualized for each member and directed toward the most appropriate, least restrictive level of care
- Identify and initiate referrals for both healthcare and community based services; including but not limited to financial, psychosocial, community and state supportive services
- Develop and implement care plan interventions throughout the continuum of care as a single point of contact
- Communicate with all stakeholders the required health-related information to ensure quality coordinated care and services are provided expeditiously to all members
- Advocate for members and families as needed to ensure the member’s needs and choices are fully represented and supported by the health care team

Required Qualifications
- Unrestricted RN license in the state of North Carolina
- 4+ years of experience working within the community health setting or 2+ years of experience providing community-based care management to members receiving long-term care, personal care services, private duty nursing, or home health
- Intermediate level of proficiency with Microsoft Office Suite
- Resident of NC - in Randolph County
- Willing or ability to travel, up to 50% of your time, in Randolph County, NC, to visit Medicaid members in their homes and/or other settings, including community centers or providers' offices
Preferred Qualifications
- Bachelor of Science in Nursing (BSN)
- CCM certification
- Experience in managing populations with complex medical or behavioral needs
- Experience working with Home Care Based Services
- Experience working with members in community and home-based settings
- Fluent in Spanish
