Job Structure, Duties, and Responsibilities
The Care Team Specialist is guaranteed 40 hours pay per week but must have FULL availability. They will have a designated two days off per week.
The Care Team Specialist is expected to lead the rest of the Caregiving Team.
They must exhibit leadership qualities by becoming an expert on company policies, adherence to the policies without fail, and by maintaining a positive can-do attitude and have a team-oriented mindset.
This is largely an action-orientated position where you will be expected to give care to a wide variety of clients. Always represent Right at Home in a positive light; outlook train, coach, and support your fellow team caregivers to reflect the Right at Home spirit.
The position will be managed and structured as:
•Schedule will be 5 days a week (to include one weekend day); available from 7:00 AM to 7:00 PM ready to work. Must be awake and ready to answer your phone by 6:00 AM on your 5 workdays, in case of a call out.
•If you are scheduled for a client’s shift, you will still be considered on call for the shift starting at 6:30 AM and after the shift until 7:00 PM.
•The Care Team Specialist will be utilized for filling shifts on short notice (on calls, new short-noticed clients, etc.) They typically don’t have a permanent client of their own, although for a short time this may occur when matching a new caregiver to a client.
•The Care Team Specialist will be part of the Retention/Leadership Team.
•The Care Team Specialist will provide hands on office training, field training for new caregivers and clients, complete client visits, care starts, reassessments, caregiver reviews, and assist with other field needs.
•The Care Team Specialist will take on back-up on call duties that are on rotation with the rest of the team.
•Conduct client and caregiver quality audits by phone, email, and in home.
•Provide new caregiver mentorships through frequent check ins by phone, email and in person.
•Care starts as needed with clients.
•Coach caregivers in client’s homes as needed or requested.
•Function as part of the leadership team by making suggestions to improve services, trainings, and company policies; report any concerns regarding the clients and the caregivers.
•Follow up HIPPA for all the client and caregivers.
•Represent Right at Home in the best light when you are with the client and caregiver, “you never know who is watching.”
•Treat each team member with respect, kindness, and dignity always.
•Always maintain a professional boundary with the caregivers and the clients.
•Complete other duties as requested.
RIGHT AT HOME's Ideal Care Team Specialist Aide will have:
•Have up to 3 years of knowledge or experience as a Certified Nursing Assistance (CNA) with an agency or in a facility setting.
- CNA/HCA License and Certification in Nurse Delegation and Nurse Delegation Diabetes, REQUIRED.
•Have knowledge of safe transfer techniques, knowledge of a hoyer lift, wheelchairs, slide boards, sit to stands, transfer poles; able to train a caregiver on these techniques.
•Maintain a professional appearance and demeanor, adhering to the company’s dress code and “good standing” at all times.
•Conduct oneself in a professional pleasant manner at all times.
•Communicate effectively in both verbal and written skills.
•Show a proactive mindset and be a self-starter. Take charge of your daily tasks and schedules.
•Ability to use a smart phone as well as a tablet/work laptop in the office.
•Take on the hard assignments, occasional off hours assignments in a time of need and attend company functions.
•Able to pass background check, drug screen, and have a clean driving record to drive clients in the company car.
•Up to date on all certifications required by DOH, DSHS, and RAH.
