Starting Salary: $141,146/yr +DOE
Schedule: Monday-Friday 40 hours/week. Occasional weekends and evenings as needed.
Job Summary: The Physician Associate/Nurse Practitioner, Mobile Health Provider is to provide a broad range of medical services for patients with the highest barriers to receiving care by taking services into the community via the Mobile Health Program (MHP). Physician Associate/Nurse Practitioner, Mobile Health Provider will deliver high-quality healthcare services in a mobile health setting, traveling to various locations to provide medical care to immigrants, refugees, houseless individuals and families, people with mental illness, substance use disorders, farmworkers, and other historically underserved communities. This role involves performing patient assessments, diagnosing and treating acute and chronic medical conditions, providing preventive care, and working closely with a multidisciplinary team to ensure the effective operation of the mobile clinic. The provider must be adaptable, comfortable with autonomous decision-making, and committed to improving access to healthcare in a non-traditional clinical setting.
Key Responsibilities:
· Direct Patient Care: Conduct comprehensive patient assessments, including medical histories, physical exams, and ordering preventive and diagnostic tests. Develop and implement appropriate treatment plans. Treatment plans may include wound care, respite care harm reduction care plans pain management, acute and chronic disease management (e.g., diabetes, hypertension), substance use disorder treatment and
referrals and coordination of care for substance use counseling, and appropriate addiction treatment programs.
· Clinical Decision-Making: Diagnose and manage both acute and chronic conditions, collaborating with other healthcare professionals as needed.
· Preventive Health Services: Provide preventive care, including vaccinations, screenings, and health education tailored to patient needs.
· Mobile Clinic Operations: Assist with setting up and maintaining the mobile
health clinic, ensuring it operates smoothly at each site. This role requires travel to various community locations, with flexible hours, including occasional weekends. The position involves working outdoors in varying weather conditions throughout the year.
· Collaboration and Teamwork: Work alongside nurses, medical assistants, and administrative staff to deliver comprehensive care. Collaborate with community partners to promote healthcare access.
· Patient Education: Educate patients on treatment plans, healthy lifestyles, and preventive health measures. Provide resources and referrals when necessary.
· Documentation: Maintain accurate and timely electronic health records (EHR) in accordance with state and federal regulations including education, medications, referrals, diagnostic testing and follow-up.
· Meet minimum patient care productivity requirements as may be set by the Center to meet requirements of outside funding and/or reimbursement sources.
· Provide continuous, comprehensive and culturally sensitive health care of the highest quality.
· Provide case management to patients.
· Provide day-to-day supervision of support staff with respect to individual patient management.
· Assist in implementation of plans to meet the objectives of the annual “Health Plan” and other goals established by the QA/QI committee and the Center’s managed care plans.
· Participate in chart audits to assess success of QA/QI program.
· Participate in medical staff, OB, QA/QI meetings and other committees within the Center established to improve patient care and the overall operation of the Center.
· Provide supervision of students and other learners in the health care professions.
· Become part of community groups or agencies to assist the Center’s integration into the community.
· Provide assistance to the Medical Director or CEO to fulfill the work of the Center.
· Perform other duties as assigned.
· Handle protected health information (PHI) in a manner consistent with the Health Insurance Portability and Accountability Act of 1996 (HIPAA).
HIPAA Requirements:
The licensed independent health care practitioners (provider) has access to PHI to create and maintain an accurate and up-to-date health record. Applying the minimum necessary standard of HIPAA, the designated record sets to which this employee will have access include all sections of the dental and medical record, patient demographic information in the practice management system, incoming records, reports, results,
consultations, etc. The [position title] should read the content of these records only to the extent needed to accomplish the assigned task (e.g. filing or disclosure).
Knowledge, Skills and Abilities Required:
· Bilingual Spanish/English preferred; willingness to become sufficiently competent in Spanish to provide medical care.
· Strong commitment to working with patients and staff from a wide range of ethnic, economic, cultural, and social backgrounds.
· Handle protected health information (PHI) in a manner consistent with the Health Insurance Portability and Accountability Act of 1996 (HIPAA).
· Valid driver’s license, reliable transportation, safe driving record and insurance coverage required.

Education and Experience Required:
· Completion of a family nurse practitioner or physician associate training program approved by the State of Oregon.
· Licensure by the State of Oregon by the Board of Nursing or the Board of Medical Examiners.
