About Administrative Investigations:
The Administrative Investigation Department plays a crucial role in ensuring compliance with policies, standards, statutes, laws and regulations to ensure integrity within the hospital's administrative operations. This position involves investigating various administrative matters, including but not limited to employee misconduct, policy violations, and operational inefficiencies.
The department utilizes Process Improvement (Lean and Lean Six Sigma), Change/Project Management and Scrum activities to identify opportunities for improvement that embrace the mission, goals, vision and values of the agency. It ensures statewide issues are addressed, resolved and all agencies are aware of the improvements made and possibly benefit them as well.
This department interacts with the community through Healthcare Partnerships. These community interactions assist with understanding the fast paced, rapidly changing healthcare environment through partnering. By partnering with colleges, universities, healthcare facilities, and other related facilities, the Colorado Mental Health Hospital in Pueblo (CMHHIP) collaborates and develops strong community partnerships. These partnerships increase communication between facilities, encourage joint ventures and sharing information. These efforts provide innovative solutions to improve quality, produce better patient outcomes, health of our staff, and increase the health and safety of the facility and community.

About the position:

Nurse Consultants provide clinical nursing expertise and technical program assistance to the Nursing Department along with all other areas at CMHHIP. Nurse Consultants will interpret the State Personnel Board Rules and regulations as they relate to nurse education and practice. In healthcare settings, registered nurses (RNs) with leadership and investigative skills play a crucial role in maintaining investigations within their departments. This position will leverage the RN comprehensive understanding of nursing department policies, procedures, and clinical expertise and apply the principles, theories, and practices of nursing to their work as an investigator. This position will operate independently in performing the full range of professional tasks and will use discretion and creativity within the limits of theory and principles of the profession, management’s program objectives, law and regulations, and general systems and guidelines. Duties in this position include:
Investigations:
- Conduct thorough investigations into allegations of employee misconduct, ethical breaches, or policy violations, analyze and interpret hospital policies, procedures, and regulations to ensure compliance and identify areas for improvement.
- Conduct thorough investigations into allegations of employee misconduct, abuse, neglect, ethical breaches, or policy violations, to include subject and/or witness interviews and audio/video evidence
- Determine the process and design the operating procedure they will follow regarding the investigation and follow regulations established by the director.
- During the investigation and after all information has been gathered, the Nurse Consultant will continue to analyze the data being gathered by utilizing Nursing theory, Nursing concepts, principles, standards of practice along with identifying policies, procedures, to reach a viable conclusion that is usable in a court of law
- Utilize their nursing skills, analysis, logic, rely on their nursing background, and couple this with nursing judgment to ensure they arrive at a sound conclusion.
- Maintain confidentiality and integrity throughout the investigative process
- Collaborate with department heads, human resources, legal counsel, and other stakeholders to gather information and evidence for investigations
- Conducts investigations in a professional manner, utilizing all available resources within the agency and according to Human Resource/Co WINS in order to successfully resolve allegations.
- Correctly logs all investigations into the report log in order to maintain accurate report documentation and tracking system.
- Completes investigative reports based on information, prior history, the seriousness of the allegation, precedence, agency guidelines, standards, regulations and state statutes. Ensures that all audio/video tapes are maintained in a secure manner and made available only to authorized personnel. Regularly monitors changes in statutes, regulations, and standards through research, notifications, and inquiries
- Correctly determines if agency policy or procedure has been violated by the actions or non-actions of any agency personnel
- Assesses relevant policies, procedures, standards. Gathers accurate, pertinent information to include agency policies.
- Successfully completes assignments to specifications, within specified timeframe, this includes gathering evidence, interviewing relevant parties, and documenting findings.
- Works closely with Human Resources, Quality Management, and other relevant departments to ensure a thorough and timely investigation.
- Assists the director in the development of policies and procedures related to investigations, compliance, and ethics to mitigate risk and promote transparency.
- Provide training and education to hospital staff on compliance, ethics, and reporting procedures to foster a culture of integrity and accountability.
- Ensures investigations are impartial and fair
- Approaches investigations objectively, setting aside personal feelings or opinions
- Scrutinize all information with a neutral perspective, avoiding any influence from personal sentiments or influence from leadership’s personal sentiments
- Base all findings on factual evidence and objective observations
- Prepare comprehensive reports of investigation findings, including recommendations for preventative measures.
- identify areas of risk, and recommend enhancements to policies, procedures, and training programs to strengthen the hospital's compliance and ethics framework. This position will also
- Serve as a resource, guiding and advisingothers on personnel actions and rules
Research:
- Utilize established Nursing theory, principles, and conceptual models, professional standards, and precedents to determine a relationship to the investigation
- Use judgment and resourcefulness when evaluating the relevance and importance of theories, concepts, and principles and tailor them in developing a plan that is best suited to the specific investigation
- Review State, CDHS, and CMHHIP policies, procedures, and license standards of practice, rules and regulations and apply relevant information to the investigation.
- Utilize Registered Nurse (RN) skills to identify deviations from established norms, protocols, standards, and workflows.
- Utilize communication, critical thinking, and decision-making abilities to conduct thorough inquiries into incidents or complaints
- Maintains knowledge of applicable rules, regulations, policies, laws, and guidelines
- Reviews and verifies documentation, ensuring that information contained therein is complete, accurate, and of a standard consistent with an official report, usable in a court of law
- Ensures confidential records are kept secure at all times
- Investigates cases that require theoretical knowledge and clinical expertise in a healthcare profession to evaluate actions within the scope of practice for that profession
- Using investigative and specialized health practice skills, researches, records and develops case files
- Gathers and preserves documentary evidence through personal interviews and records from employers, patients, other regulatory agencies, or police
- Recognizes issues that require knowledge of health care principles and practices to elicit information and obtain clarifications
- Reviews licensure, performance evaluations, personnel file, competencies, and completed training for each staff member being investigated
Report Findings/ Recommend Outcomes:
- Analyze complicated sets of data and/or documents, interpret hospital policies, procedures, and regulations and apply that information to recommendations given to stakeholders
- Document findings, prepare detailed investigative reports, and present findings to relevant stakeholders
- Recommend and implement corrective actions, policies, or procedures to ensure compliance and address identified issues, areas for improvement, and to prevent future occurrences.
- Receives preliminary reports then analyzes, interviews, collects necessary information, compiles information, and prepares the report for the Appointing Authority
- Ensures Appointing Authority is aware of licensure issues immediately
- Explains the investigation process and possible actions the Appointing Authority may take.
- Alerts Appointing Authority of issues/factors identified in the investigation.
- Advises the Appointing Authority regarding alleged violations of the practice acts, providing documentary evidence compiled through investigations
- Compiles information and writes reports of investigations, explaining health care practice issues.
- Documents evidence and statements of witnesses in a manner adequate to support administrative actions brought by the Appointing Authority. Violations are documented in the report and submitted to the Appointing Authority consistent with agency policy
- Testifies at hearings

Minimum Qualifications:
Education/Licensure/Certification/Experience:
Master’s degree from an accredited institution in Nursing
Current, valid licensure as a Registered Nurse from the Colorado Board of Nursing or any state participating in the Enhanced Nurse Licensure Compact (eNLC)
Three (3) years of professional experience to include conducting investigations, compliance with regulations (federal, state, and agency-specific policies), compliance with accrediting agency standards, and understanding current nursing standards
Substitutions
Additional appropriate experience will substitute for the degree requirement on a year-for-year basis
Preferred Qualifications:
- Previous State Experience
- Have been an Appointing Authority for at least 1 year
- Previous experience as a nursing supervisor who supervised one or more units (20+ staff) for 5+ years

- Former State employees who were disciplinarily terminated or resigned in lieu of termination must (1) disclose that information on the application (2) provide an explanation why the prior termination or resignation should not disqualify the applicant from the current position, (3) provide the employee number from the applicant’s prior State employment. Absent extraordinary circumstances, prior disciplinary termination or resignation in lieu of termination and failure to provide this information will disqualify the applicant from future State employment with CDHS
