Supervise: This position supervises, leads, trains, and coaches professional, clinical, and support personnel.
The Director of Nursing provides overall management of Allegiant Home Care’s clinical programs related to the coordination of all professional and paraprofessional services provided to patients in accordance with its policies and procedures.
Qualifications and Requirements:
- Current Florida licensure as a Registered Nurse
- Current driver’s license and dependable insured automobile
- Educational preparation Bachelor of Science in Nursing (BSN), preferred
- Graduate of an accredited School of Nursing
- Minimum three years clinical experience within the last five years, preferably in-home health care and at least one year of supervisory experience
- Current CPR certification
- Level II ACHA background screening
- Possess knowledge of home health care regulations and procedures
- Possess effective communication skills and ability to lead a team of personnel with a variety of backgrounds and educational experiences
- Possess good time management, problem-solving, work ethic skills
Skilled at:
- Strong communication skills with ability to listen
- Ability to work autonomously yet support the needs of Allegiant Home Care and your fellow team members
- Computer proficiency with Microsoft Office including Word, Excel, Outlook, and PowerPoint
Medical Clearance
- Current TB testing and immunizations/declination for Hepatitis B and Flu

- Work Environment: Employee must be able to work effectively within an interdisciplinary team model, interfacing and collaborating with a wide range of clinical and social services disciplines who work together to manage the Allegiant Home Care patients' care. The work setting is in a variety of environments including patient homes, Assisted Living facilities, hospitals, and in their own vehicle. The environment can fluctuate with the outside weather as well business locations, as well as patient homes, which can provide extremes in the environment.
- Physical Requirements - Requires manual and finger dexterity and eye-hand coordination; the ability to use department equipment; and the ability to lift/carry up to 30 pounds using appropriate body mechanics with reasonable accommodation if needed. Must be able to push, pull, stand, bend, scoot, and sit. Must be able to assist patients up to 300 lbs. to stand, sit, or roll over.
- Visual, Hearing and Communication Requirements - Requires corrected vision and hearing to normal range, with or without reasonable accommodation. Ability to communicate by way of the telephone with patients, customers, vendors, and staff.
- Pressure Factor - Requires working under stressful conditions. Working conditions may be noisy and crowded, with fluctuating indoor temperatures. Moderate pressure to meet scheduled deadlines. Potential for exposure to verbal aggression by patients, vendors, and or employees.
- Environmental Conditions - May be exposed to a risk of bodily injury through contact with moving instrumentation, toxic substances, medicinal preparations, bodily fluids, communicable diseases, and any other conditions common in a healthcare environment. Subject to unpleasant odors.
Duties and Responsibilities:
- Demonstrates support for the agency mission statement and philosophy to promote quality, efficient, comprehensive, compassionate, and effective patient care
- Supervises and serves as a role model and consultant to assigned agency personnel
- Facilitates initial orientation, competency testing, and in-service training for new and existing personnel
- Ensures that patient and personnel safety, emergency preparedness, and infection control practices are included in orientation and continuing education
- Provides oversight to ensure the coordination of referrals and admissions
- Provides oversight to ensure the development, implementation, and update of the individualized patient plan of care
- Monitors the coordination of and promotes the interdisciplinary group process
- Maintains current knowledge of home health care regulations and standards
- Coordinates clinical services with other programs and managers
- Follows-up on patient complaints and grievances using effective communication
- Maintains confidentiality of patient, employee, and agency operations
- Keeps all records up to date, organized, and ready for inspection at any time
- Ensures overall development, monitoring, and implementation of the QAPI plan including the annual evaluation
- Co-Chairs agency level QAPI meetings and ensures minutes are taken and maintained
- Coordinates and reports on performance improvement activities and outcomes at the agency level and serves on the Compliance Quality Management Committee and Compliance Quality Oversight Committee
- Reviews and ensures all incident reports for accuracy, completeness, and risk level, and follow-up needs
- Implements processes to monitor and evaluate trends and coordinates/prepares trend reports on safety, risk management, adverse events, infection control, outcomes, patient, physician, referral source and personnel satisfaction and grievances, and clinical record documentation of service provided
- Coordinates and/or performs clinical record reviews, identifies trends, and reports on record reviews bi-monthly monthly
- Ensures the development and implementation of policy and procedure and coordinates the annual review of policy and procedure
- Participate in organizational committees as identified that enhance the working relationships between departments within Alivia Care, Inc.
- At all times, represent Alivia Care, Inc. professionally through care and/or services provided to all clients and patients
- Comply with Alivia Care, Inc. policies, procedures, and standard practices
- Maintain the confidentiality of patients and families within Alivia Care, Inc. as per government mandates
- Use resources in a fiscally responsible manner
- Promote Alivia Care, Inc. and Allegiant Home Care through participation in community and professional organizations
- Improve own professional knowledge and skill level
- After-hours work including rotating schedule of nights, weekends, and holidays
