Your Role as Lead MDS Director

As the Lead RN/LPN MDS Director, you will be a cornerstone in managing and coordinating resident assessments, care planning, and interdisciplinary teamwork. You will ensure compliance with federal and state regulations while supporting a high standard of care.

Key Responsibilities Include:

Overseeing the development and completion of MDS assessments and care plans to meet regulatory requirements.
Ensuring timely submission of assessments to the State repository per RAI Manual guidelines.
Collaborating with the Interdisciplinary Care Team to tailor care plans to each resident’s unique needs.
Monitoring Quality Measures Reports to enhance outcomes and support our Five Star Rating.
Providing education and support to residents, families, and care staff about care planning.
Participating in facility surveys and government inspections, ensuring compliance and readiness.

Your leadership will empower the care team to provide personalized, high-quality care while maintaining operational excellence.

What You Bring to the Role

We’re looking for an experienced and compassionate leader ready to inspire teams and elevate care standards. If you meet the following qualifications, we encourage you to apply:

Active RN or LPN License: Current and unencumbered in Florida.
Experience: Minimum of 2 years in a healthcare setting (hospital, nursing facility, or similar).
MDS Expertise: Knowledge of MDS assessment processes, care planning, and quality measure monitoring.
Leadership Skills: Strong ability to motivate and guide interdisciplinary teams.
Regulatory Knowledge: Familiarity with federal and state guidelines for skilled nursing facilities.
Personality: A blend of compassion, tact, and enthusiasm with a proactive approach to problem-solving.
