Patient Navigator (RN) - Oncology

The RN Patient Navigator (NPN) helps coordinate services among medical personnel. Navigators provide support and guidance, linking patients to existing health care services. The NPN is a nurse who is familiar and experienced with oncology care and will be physically available to the patient and family. The NPN acts as clinician, care coordinator, educator and counselor. The NPN works collaboratively within an interdisciplinary health team and is responsible for providing comprehensive and age-appropriate care to both well and/or ill patients. The NPN educates patients and/or intervenes in acute and chronic illness.
Education

Associate's degree from an accredited school of nursing required
Bachelor's degree in Nursing preferred

Experience

3-4 years experience providing direct patient care in an oncology or related clinical setting required

Licenses and Certifications

RN - Registered Nurse - State Licensure and/or Compact State Licensure in Maryland required
CPR - Cardiac Pulmonary Resuscitation (includes BLS and NRP) for healthcare providers from either the American Heart Association (AHA) or American Red Cross within 90 Days required
Additional unit/specialty certifications may vary by department or business unit.
OCN - Oncology Nurse Certification or infusion certification (CRNI) within 180 Days required

Contributes to the achievement of established department goals and objectives and adheres to department policies, procedures, quality standards and safety standards. Complies with governmental and accreditation regulations. (STAFF LEVEL)Based on the assessment of physical, emotional, bio-physical, and spiritual needs, provides direct patient care and evaluates outcomes. Incorporates the principles of patient-centered care.Demonstrates knowledge of current literature in at least two (2) areas of practice, eg: critical care, clinical practice, administration, disease processes.Designs, implements, evaluates, and documents individual health teaching for patient and/or significant others. Improves patient outcomes through the use of education, support, and monitoring.Facilitates use of agency and community resources. Facilitates communication between patient/family and interdisciplinary team to include: pharmacist, dietician, social worker and counselors, among others. Serves as a resources person in planning and implementation of care for complex patients within the specialty area through access to existing community services.Participates in monthly Cancer Committee/Tumor Bard conferences.Participates in studies, special projects, and research activities as appropriate. Aware of current open clinical trials available to patients. Management of patients on clinical trials.Prevents, minimizes, and reports symptoms/toxicities/health abnormalities related to disease or treatment to the physician and other allied personnel. Involves the PCP in the patient care plan as appropriate.Provides psychological support to patients and families. Provides supportive care throughout the disease process: diagnosis, treatment, rehabilitation, unexpected complications/setbacks, palliative and terminal care.
