General Summary:
The Licensed Practical Nurse (LPN) provides professional nursing care to patients within patient care services on medical-surgical units. Professional nursing practices, requiring specialized knowledge, judgment, and skill are performed in accordance Bayhealth policies. After competency verification, the Licensed Practical Nurse may deliver patient care services to the adult, and geriatric populations. The care may include, but is not limited to, the following: obtaining and recording in the medical record: blood pressure, pulse, temperature and pulse oximetry, drawing blood, EKG, administration of oral medications, administration of maintenance fluids, administration of tube feedings, transporting patients, obtain urine and stool specimens, perform point of care testing, assist with procedures consistent with the scope of the employee’s position (i.e.. dressing changes, urinary catherization, bathing, dressing, grooming, toileting, ambulating, transferring, feeding, observing, and reporting the general well-being of the person(s) to whom they are providing care).
Responsibilities:
- The LPN must read and interpret documents such as doctors’ orders, medication names, and lab tests. Perform focal assessments for established inpatients.
- Provide general basic nursing care, such as positioning patients, lifting and turning, assisting in use of bedpan, commode, urinal or bathroom, and weighing.
- Provide exceptional customer service that aligns with the Planetree philosophy.
- Perform routine, uncomplicated treatment procedures including enemas, application of hot and cold compresses, dressing changes, wound cleansing, obtaining vital signs including orthostatic vital signs and rectal temperatures and splinting of extremities. Collects and records intake and output.
- Prepare and administer oral medications as ordered by the physician or APC. Administer maintenance intravenous fluids per physician order, under the direction of the primary care Registered Nurse. Review medication history with patient.
- Assist with minor medical/surgical procedures in the practice setting.
- Performance of venipuncture for the purpose of blood and to monitor the patient’s condition before, during and after the withdrawal of blood.
- Transport patients using wheelchair or stretcher within hospital and to/from vehicles. Assist those patients requiring help with ambulation.
- Identify the need for patient education and advise patients on their treatment plan.
- Demonstrate the ability to recognize abnormal lab results.
- Document treatment records for each patient. Immediately report any observed changes in condition or behavior of patients and documents any care given in an acceptable medical legal fashion. Immediately reports any unusual incidents.
- Set up for suturing and procedures, using sterile technique; dismantles procedure set, caring for soiled instruments safely and correctly.
- Maintain patient confidentiality; comply with HIPAA and compliance guidelines established for the practice.
- Ensure compliance with Joint Commission regulations and other safety requirements; this includes disposal of used sharps and contaminated materials, as well as, safe storage of medications.
- Re-order supplies as required and within budgetary limitations. Maintain work station cleanliness and restock linen and supplies to exam rooms; fills blanket warmer and IV trays; provides general cleaning, including emptying trash, straightening waiting area, etc. Washes and changes stretcher or exam table linen and empties dirty laundry.
- Demonstrate the ability to reflect knowledge of current professional practice standards, laws and regulations, evaluate their own nursing practice in relation to professional practice standards, relevant statutes and regulations. Engage in an annual self-assessment, identifying areas of strength and areas requiring professional development Seek and reflect on constructive feedback from other members of the healthcare team. Acts to achieve performance goals.
- Acquire and maintain current knowledge and competency in nursing care. Participate in ongoing educational activities to acquire and validate knowledge (i.e. BLS, NAPPI). Seek experiences that reflect current clinical competencies and new clinical information. Maintain professional affiliations. Attend regularly scheduled staff meeting. Perform all other tasks and projects assigned by the Charge Nurse/ Clinical Coordinator. May be required to act as back-up for other members of the staff.
- Interact with and contribute to the professional development of peers and other healthcare providers. Share knowledge, skills, and experience with colleagues and others. Provide colleagues and peers with constructive feedback regarding their practice. Contribute to a learning environment that is conducive to healthcare education. - All other duties as assigned within the scope and range of job responsibilities.
- Assisting physician/surgeon by scrubbing in for Operating Room cases, when properly trained, if applicable.
Required Education, Credential(s) and Experience:
- Education: Certificate Program
- Licensed Practical Nurse
- Or equivalent credit hours in an RN program.
- Credential(s): Basic Life Support Licensed Practical Nurse
- Experience: Required: New graduates may be considered
