The Comprehensive Stroke Center (CSC) Stroke Coordinator is responsible for coordinating and implementing services and activities as it relates to the Hospital of the University of Pennsylvania (HUP) Comprehensive Stroke Center. The stroke coordinator works amongst a large interdisciplinary team to collaborate and support the advancement of stroke care across the population and continuum of care. This individual will be responsible, in partnership with the Stroke Medical Director, for developing, implementing and monitoring stroke center initiatives and will play a lead role in the development and implementation of outcomes evaluation as guided by the Joint Commission accreditation standards. In doing so this role will function as a liaison between physicians (across and within departments), nursing staff, allied health care professionals, patients and their families. In addition, the CSC Stroke coordinator will participate in program development as it relates to the Comprehensive Stroke Center. This includes -but is not limited to staff education, community education, EMS partnership and marketing.
We are looking for a nurse with a high level of autonomy, flexibility, & critical thinking. A background in quality and neurology are preferred.
Summary:
- The Comprehensive Stroke Center (CSC) Stroke Coordinator is responsible for coordinating and implementing services and activities as it relates to the Hospital of the University of Pennsylvania (HUP) Comprehensive Stroke Center. - The CSC Stroke Coordinator will also play a key role in the Neuroscience Service line, as it relates to the development of uniform and coordinated stroke care across the system hospitals. In this capacity this individual will partner with system peers and help to lead and support the planning and implementation of evidence-based clinical care pathways and initiatives which support the advancement of stroke care across the population and continuum of care.
Responsibilities:
- Oversight for maintaining, in collaboration with the Stroke Medical Director, the Comprehensive Stroke Center Mission, Vision and Scope of service and target population.
- Adheres to a Comprehensive Stroke Center population health approach by participating in the design, implementation and evaluation of care, treatment and services related to stroke care throughout the stroke continuum of care. Evaluates new stroke care practices in the setting of research findings. Tests and implements newly available theory and research findings in clinical settings or hospital systems appropriately.
- Provides for the uniform performance of care delivery using evidence-based practice standards. Facilitates the development of practice guidelines that are selected based on relevance to the population served and current evidence. Oversight for development and implementation, in collaboration with the Stroke Program Medical Director and PennMedicine CEQI, of the stroke performance improvement program as well as an interdisciplinary collaboration necessary for clinical excellence.
- Facilitates the access and utilization of the hospital and community resources to assist in patient’s recovery and ongoing screening of health risk factors.
- Coordinates the American Heart Association Get with the Guidelines® Stroke Database and personnel necessary to abstract, upload and report outcomes. Assures capture is timely and accurate. Collects additional data as necessary from clinical partners including but not limited to, Neurosurgery, Vascular Surgery, Emergency Department, Penn Star and Rehabilitation. Analyzes data, prepares periodic reports and makes these available to Clinical Effectiveness Teams, Comprehensive Stroke Center Committee and the Neuroscience Service Line as appropriate.
- Assures that the care team members function within their scope of practice, certification and licensure and competency. Collaborates with all members of the health care team including physicians, nurses, therapists, home health care/facilities and community services to promote team building and provide clinical consultation and education.
- Establishes a presence of leadership and develops effective working relationships with administration, Neuroscience service line, medical and nursing staff and other hospital staff. Serves as a role model and stroke expert, providing clinical consultation and education for the members of the multidisciplinary team – both internal and external to the acute care hospital – related to care delivery, expected patient outcomes, and resource availability. Coordinates and prepares the hospital and staff for TJC Disease Specific Stoke Certification Surveys.
- Participates in community education and outreach by developing and implementing education programs to promote stroke awareness. Acts as a resource to Penn Medicine’s network affiliates and the community.
Required Credentials:
- Registered Nurse - PA (Required)
- BLS Certification through the AHA (Required0
- Stroke Certified Registered Nurse - within 2 years of starting the position (Required)
Required Education or Equivalent Experience:
- Bachelor's Degree AND 5+ years clinical experience in neuroscience nursing (Required)
- BSN (Preferred)
