Intely@Sea Course Overview – Incident Reporting and Defensive Documentation

Intely@Sea is a five-day cruise out of Miami, Florida to the Bahamas, taking place May 9 – 13, 2022 in honor of Nursing Professionals Week! We are so excited to be a part of this celebration. It is going to be a trip to remember! 

Lynn Barry, MSN, RN, and Director of Clinical Quality and Compliance at IntelyCare, will teach a course for nursing professionals on incident reporting and defensive documentation during Intely@Sea.  

What is Incident Reporting and Defensive Documentation? 

Unforeseen conflicts often arise in the care setting between nursing professionals, facility administration, patients, and/or patient families. When these conflicts arise, it is just as important to properly document the incident as it is to diffuse the incident itself.  

The goal of incident reporting is to improve patient safety by gathering and accurately documenting information. By providing a detailed account of the chain of events leading up to and following an unforeseen incident, nursing professionals and their facilities can prevent future occurrences that threaten patient safety. In that respect, incident reporting is also a powerful source of information for nursing professionals, fellow caregivers, patients, and facilities in maintaining safety and preventing further unforeseen incidents.  

Course Objectives 

Lynn’s Intely@Sea course will examine how to properly conduct incident reporting using defensive documentation best practices, and how documentation is an event itself. 

Upon completion, nursing professionals will have the knowledge and framework to accurately report incidents in a way that prevents unneeded regulatory action on the nursing professional, facility, or any other party involved. Understanding and utilizing best practice of accurate defensive documentation will help avoid allegations of misconduct by way of misinformation.

The course will examine real examples of patient care and use lessons learned to vastly improve incident reporting and documentation.  

Learning Outcomes 

  • The learner will identify the benefit and need of incident reporting.
  • The learner will identify best practices for documenting defensively.
  • The learner will demonstrate knowledge through case studies and documentation samples.
