Procedure for Making Claims of Copyright Infringement

IntelyCare, Inc. is registered with the United States Copyright Office as a Service Provider (pursuant to the “DMCA,” the Digital Millennium Copyright Act 17 U.S.C. § 512). If you believe that your work has been copied in a way that constitutes infringement on IntelyCare’s websites (“Sites”) or the services available through them (“Services”), please provide the information below to IntelyCare’s registered agent. For any notice of claimed infringement, be sure to include the following information, all as required by the DMCA:

  • A physical or electronic signature of a person authorized on behalf of the owner of an exclusive right that is allegedly infringed;
  • Identification of the copyrighted work claimed to have been infringed or to be the subject of infringing activity and that is to be removed or access to which is to be disabled;
  • Information reasonably sufficient to permit IntelyCare to locate the material at issue (specific url if available is best);
  • Information reasonably sufficient to permit IntelyCare to contact the complaining party such as an address, telephone number, and if available, an email address at which the complaining party can be contacted;
  • A statement that the complaining party has a good faith belief that use of the material in the manner complained of is not authorized by the copyright owner, its agent, or the law; and
  • A statement that the information in the notification is accurate and, under penalty of perjury, that you are authorized to act on behalf of the owner of the copyright that is allegedly infringed.

IntelyCare reserves the right to delete or disable content alleged to violate copyright laws or the applicable terms covering use of the Sites and Services. IntelyCare will terminate the account of copyright or terms violators.
